ATTACHMENT VIOLATION:
Technical Specification 6.2.A requires that the applicable procedures recommended in Appendix "A" of Regulatory Guide 1.33, Revision 2/5 February 1978 be maintained and adhered to.
Section 8(b)(2)(bb) of Appendix "A" requires procedures for calibrations of process radiation monitors.
P LaSalle Procedure (LCP-820-13) requires that the solid cesium-137/
standard source response recorded in Attachment A of LCP-820-13, be within<
plus or minus 20 percent of the decay-corrected initial calibration response.
- j contrary to the above, for the calibration of the "B" RHR liquid process radiation monitor performed in 1986, the solid cesium-137 standard source response, recorded in Attachment A of LCP-820-13, was not within plus or minus 20 percent of the decay-corrected initial calibration response.
Specifically, the standard source response differed from the decay-corrected initial response by 24%.
CORRECTIVE ACTION TAKEN AND RESULTS ACHIEVED On July 8, 1987, the portion of LCP-820-13 that checks the detector response to a cesium-137 solid source was performed. The detector responded to within 8.5% of the expected response, well within the +/- 20% criteria (which if exceeded requires recalibration).
CORRECTIVE ACTION TAKEN TO AVOID FURTHER VIOLATION A tailgate was held for the chemistry group on August 24, 1987.
This tailgate discussed this event and re-emphasized that if a procedure step is encountered where there is uncertainty on the performance, contact supervision for direction. New personnel will continue to have on-the-job training on calibration procedures to be performed for the first time. This l
on-the-job training will ensure a thorough knowledge of the acceptance criteria and appropriate corrective actions to resolve discrepancies.
The procedure was reviewed for adequacy and clarity and found to be satisfactory.
