Areas Insoected: A routine unannounced inspection of an in-air wet source-storage irradiator. The inspection consisted of an examination of the facilities and safety related interlocks, a review of procedures and representative records, interviews with personnel and measurements performed by the inspectors.

Results: No apparent violations were identified. .)i i

2. Organization and Licensee Internal Audits The Compliance Officer is responsible for regulatory affairs including licensing and inspection related correspondence. An outside consultant l

performs annual audits of the radiation safety pra ram. The last such I audit was performed on October 5, 1985. A report of this audit was filed with the Commission on October 8, 1985.

No violations of NRC rules, regulations or license conditions were identified.

3. 4. Radiation Protection Procedures Operating procedures are dictated by the computer program which controls the operation of the irradiator. Procedures must be followed in a precise sequence to allow the operator to raise the source. The safety interlock systems are continuously monitored by the computer to determine that they are operational. Emergency procedures are posted and were followed by operators when system malfunctions were identified by the computer irradiator controller.

5. Materials, Facilities and Instruments Facilities were as described in the license application. A seismic switch was installed and calibrated by an outside consulting firm. Survey instruments were operating and calibrated. The area radiation monitor was functioning properly.

6. Control of High Radiation Area' The entry to the irradiator cell is controlled by a computer program which will not allow the doors to be opened except when the source is in the shielded position and radiation monitors indicate normal low radiation levels. The source can not be raised with either or both doors open.

Operational procedures require that the operator " sweep" the cell to ensure that no individuals remain in the cell area before raising the source rack. Emergency door switches were operational. The level control for the liquid shield was operating and low water levels the week before the inspection resulted in an alarm being signalled and the control system i being put on alert status until water was added to the pool, i

7. Personnel Protection Personr.el dosimetry records indicated minimal exposures. Surveys by the inspectors showed levels of radiatior, less than 50 micr; rem per hour in all unrestricted areas confirming the licensee's surveys. Leak tests of sealed sources were performed and indicated no leaking tources.

8. Effluent Control and Waste Discosal Slightly contaminated packaged ion exchange resins were stored on the roof of the irradiator cell. The exposure reading on contact was less than 40 microrem/hr. No radioactive waste material has been released from the I I

facility.

9. Exit Interview The results of the inspection were discussed with individuals indicated in Section 1.

At no time during the inspection was written material given to the licensee by the inspectors.

During the telephone discussion between Mr. Gene Nestor of your staff and i Mr.- James H. Joyner of this office on August 5,1987 concerning the' NRC .l inspection perfomed at your. facility on-July 23 and 24,1987 by Ms. Susan l Billhorn and Messrs. John White and John Miller of- our office, Mr. Joyner {

expressed our concern about the current rate of. water loss from your '

irradiator. pool. Since the water loss rate indicates that the pool is leaking, we are concerned that, should any of the cobalt rods begin leaking in the future, a release path to the environment could exist. Accordingly, based on this discussion, we understand the following:

1. Effective July 24, 1987, you began daily monitoring of the water level in

- the irradiator pool. Such monitoring consists of recording, in the operating log book, the water level before and after the addition of makeup water. All data collected through September 1,1987 will be promptly submitted to the NRC Region I office along with your assessment i of the cause of the loss and your proposal for resolving tlee loss of pool water.

2. Effective July 24, 1987, you began daily monitoring of the pool water for.

the presence of radioactive contamination. Such monitoring and recording of the results is in accordance with your established procedures.

3. Promptly notify the NRC Region I office by telephone of any decision to terminate activities involving materials authorized by-your NRC license.
4. Conform with the requirements of 10.CFR 30.36(b) relative to imediately infoming the Comission in writing of any decision to teminate activities involving materials authorized under your NRC license.

Issuance of this Confirmatory Action Letter does not preclude the issuance of an Order formalizing the above commitments.
