INSTRUCTIONS Introduction This fom is intended as a tool for the observer to use in observing and evaluating an exercise. There are several different " modules" corresponding to different observer assignments; an observer assigned to V

the EOC should have the "EOC Module", an observer ass %ned to the Emergency News Center should have the " Emergency News Center Module", and so on. The module you have should correspond to your assignment.

You should bring the fom along with you when;rou go to observe. The questions in it will serve as a useful guide concerning what go.look for.

You may wish to fill out some or all of the questions while e1 ocation.

l NOTE: Some responses may already be marked as N/A because the e.ctivity is rated under another part of the response organization. [

Directly below is a short explanation of % hat is in the fom and how to fill it out; please read this explanation as soon as possible.

Concept of Form and Instructions, for Use (1) Structure and Fonnat.

Eac5 module is composed of several sections.

Each section is about a particular part of the i

emergency activities - for example, "equipawnt,"

.. +'

" communications," or " dose assessMnt." Eacn section has two parts.

The first part is a series of quastions for you to rate the perfonnance. The second part is a sunnary section, wita j space to write a narrative essay summarizing your overall impressions.

(2) Rating Questions. You have four options when answering 'these' questions:

Satisfactory, Ltsatisfactory, N/A, and N/0.

l

" Satisfactory" means that personnel and equipment performed

)

o at an acceptable. level.- Errors noted were not severe and the task was completed in an acceptable otanner. For example,' if personnel are to check thet r dotimters oncq every 15 minutes, but the person your we observ!ng checks i

his dosimeter pnly 7 of the 8 times he should in 2 hours2.314815e-5 days
5.555556e-4 hours
3.306878e-6 weeks
7.61e-7 months
, l

his performance is satisfaction. However, your narrative summary should explain the rating, o

" Unsatisfactory" means that personnel and equipment generally perfonned below an acceptable level. There were deficiencies of a significant nature. The area's ability to carry oup its function was significantly impairet
