Gentlemen:
This letter provides our response to the Notice of Violation contained in Reference (b).
The Notice of Violation has been repeated below for completeness.
NOTICE OF VIOLATI.QN Technical Specification 5.12.2 requires, in part, that locked doors shall be provided to high radiation areas in which the radiation intensity is at such levels that a major portion of the body could receive in any one hour a dose in excess of 1000 mrem to prevent unauthorized access to those areas.
Contrary to the above, on June 17 and June 25, 1987 a door located in the upper level of the Spray Building, which controls access to the lower levels of the building where whole body radiation levels exceed 1000 mr/hr, was not in a locked condition.
The above instances of failure to adequately control access to a locked high radiation area constitutes a Severity Level IV Violation (Supplement IV).

l Corrective Steos Taken and Results Achieved When. informed by the NRC. resident inspector on June 17, 1987 that the Spray Building door was unlocked, personnel were immediately dispatched to verify the door had been relocked.
Investigation revealed that the door had been checked at 0500 hours0.00579 days <br />0.139 hours <br />8.267196e-4 weeks <br />1.9025e-4 months <br /> on June 17, 1987 and found locked.
It was discovered that operations personnel had exited the door later that day.
Since the door was fitted with an automatic closer, the operator assumed the door was properly latched when he exited.
All plant personnel were reminded of the need to verify that doors are locked upon exiting such areas.
A Discrepancy Report (DR)'was issued on June 18, 1987 to check the locking and closing mechanisms on the door.
Repair efforts were completed and the door was satisfactorily tested by contractor personnel on June 25, 1987.
When the same door was.found ajar on June 25, 1987, further investigation revealed a slight bow in the door frame which prevented the door from consistently properly latching.
Repairs to the door frame were completed on June 30, 1987.
There have been no further instances of improperly latching high radiation atJa door locks subsequent to June 25, 1987.
Corrective Step 1_Iden to Avoid Further Violations All locked doors to high radiation areas as required by Technical Specification 5.12.2 were checked for proper operation.

i Discrepancy Reports were written for any improperly locking door closures. These doors were placed on increased surveillance checks until
'a repairs were completed.
Procedure 9.1.6 was revised to require Radiological Controls personnel to verify proper operation of the closing mechanism each time the locked door surveillance is performed.
