MONTHLY OPERATING ACTIVITIES - MARCH 1987 HIGHLIGHTS The reporting period began _with the unit in Mode 1 at the 80% power limit administrative 1y imposed to minimize steam generator tube degradation. A unit shutdown began on 3-3-87 due to increasing primary to secondary leakrates. A reactor trip occurred from less than 1% power due to a high spike on the N-31 source range detector. The reporting period ended with the unit in Mode 5 at half-loop.

Estimated start-up date is April 15, 1987.

Gross Electrical Generation for the month of March was 51,850 MWH.

SUMMARY

3-1-87 The reporting period began with the unit in Mode 1 at 80%

power.

3-3-87 At 0517, a unit shutdown was started due to increasing primary.

to secondary leakrate measuring.302 gpm.

At 1440, the generator was taken off line, Mode 2 was entered.

At 1505, all control rods were inserted.

At 1507, shutdown bank D rods were inserted.

At 1511, a reactor trip occurred from less than 1% power due to a high spike on N-31 source range detector.

3-4-87 At 0237, Mode 4 was entered.

At 1416, Mode 5 was entered.

3-10-87 At 2100, in Mode 5, half-loop was achieved.

St. Amand TELEPHONE (616) 465-5901 PAGE 1 of 1 MAJOR SAFETY-RELATED MAINTENANCE MARCH, 1987 M-1 The #4 Boric Acid Transfer Pump was ound leaking at the flange and shaft seal. The pump was rebuilt, orified to operate correctly, and returned to service.

M-2 Several component cooling water system leaks from cracked welds were found and repaired.

M-3 100 percent eddy current examinations were performed on the steam generators.

110 tubes were plugged based on the results of the testing.

-M-4 The Steam Generator manway bolt hole threads were found to be damaged upon removal of the manway covers. Repairs were completed by Westinghouse.

M-5 Rod L13 of Shutdown Bank "C" did not move during rod insertion.

Investigation showed a faulty connection at disconnect #8 on board assembly #4. The connector was disassembled and inspected, no abnormalities were found. The connector was reassembled and verified to work properly.

M-6 Essential service water to East Auxiliary Feed Pump Valve 2-WM0-754 failed to close properly when being cycled for valve testing.

Inspection showed the stationary block for the Torque switch loose, and the open limit switch incorrectly set.

Both problems were repaired and the valve verified to operate correctly before returning to service.

16C-1 Channel 1ofg-31,sourcerangedetectorwasfound ingicating10 counts per second while N-32 channel 2 was indicating 10 and decreasing.

Investigation showed the detector for N-31 to be defective. The detector was replaced and calibrated prior to returning it to service, u

I&C-2 The Ammeter for the intermediate range nuclear instrumentation was observed not working properly. The ammeter was i

found to be defective. Replaced' ammeter, performed calibration, and performed surveillance test prior to returning to service.
