APPENDIX A NOTICE OF VIOLATION Louisiana Power & Light Company Docket:
50-382 Waterford Steam Electric Station, Unit 3 License:
NPF-38 During an NRC inspection conducted July 16 through August 15, 1987, a violation
]
of NRC requirements was identified.
This violation involved failure to follow I
an approved maintenance procedures.
In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), this violation is listed below:
Technical Specification 6.8.1.s requires, in part, that written procedures be implemented for the applicable procedures recommended in Appendix A of l
I Regulatory Guide 1.33, Revision c February 1978.
Procedures for maintenance that can affect the performance of safety-related equipment are recommended in paragraph 9.9 of Appendix A of Regulatory Guide 1.33, Revision 2, Febrmry 1978.
Procedure MM-12-001, Revision 1, " Pipe Hanger Support Installation, Fabrication, and Removal,"
l 1s an approved procedure for maintenance that can affect the performance of safety-related equipment.
Step 5.9 of Procedure MM-12-001, Revision 1, requires that when threaded' fasteners are used, locking devices shall be installed. Attachment 10.1 of Procedure MM-12-001, Revision 1, requires that when locknuts are installed, the locknuts must be flush with the bolt.
Contrary to the above, on August 8,1987, Pipe Supports SIRR-3006 and

SIRR-3008 were found to have threaded fasteners without locking devices l
installed.
On August 11, 1987, Support MSSH-3084 was found to have threaded fasteners without locking devices installed, as well as others with locknuts installed that were not in contact with the bolt.
This is a Severity Level IV violation.

Pursuant to the provisions of 10 CFR 2.201, Louisiana Power & Light Company is hereby required to submit a written statement or explanation to this Office within 30 days of the date of the letter transmitting this Notice. This reply, should include for each violation:
(1) the reason for the violation if l
admitted, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
