This refers to your letter dated February 9, 1987, requesting amendment of your byproduct material license for use in medical teletherapy. We have completed review of your application and have the following comments and need j for additional information, t

1. The Statement of Training and Experience and Preceptor Statement for Dr. Markette does not indicate that this physician has received training and/or obtained experience as identified in 10 CFR 35.960 (enclosed).

Specifically, this physician must be board certified as described in S35.960(a) or have received 200 hours0.00231 days <br />0.0556 hours <br />3.306878e-4 weeks <br />7.61e-5 months <br /> of classroom and laboratory training and 500 hours0.00579 days <br />0.139 hours <br />8.267196e-4 weeks <br />1.9025e-4 months <br /> of supervised work experience as described in S35.960(b).  ;

2. Since your license will be issued in conjunction with the recently amended 10 CFR Part 35, you must identity your teletherapy physicist and describe his training in accordance with S35.961.

If we do not receive a reply from you within 30 calendar days from the date of this letter, we shall assume that you do not wish to pursue your application /
