FITNESS FOR DUTY PROGRAM

SUMMARY

1 The Waterford 3 annual random testing rate is 50 percent. During this reporting 4

period, approximately 26.7 percent of the population have been randomly tested.

One positive test result occurred during random testing that was associated with a licensee employee collection. Four other positive test results occurred during pre-access testing of short-term contractor personnel. The rate of positive tests from random testing is 0.3 percent.

POSITIVE RESULTS:

There were five positive test results for this reporting period. Three of the five positive l

tests were for marijuana and the other two positive tests were for cocaine. The Federal l

Regulation cut-off for marijuana and cocaine is 100 ng/mi and 300 ng/ml, respectively.

Entergy, however, uses a more stringent cut-off criteria of 50 ng/ml for marijuana. As illustrated in the breakdown of confirmed positive tests, marijuana is the drug of l

preference, accounting for 60 percent of the positive results and cocaine is second, yielding 40 percent of the positives. These results are consistent with previous l

submittals from Waterford 3.

MANAGEMENT ACTIONS:

l Access was denied for the four short-term contractor personnel that tested positive during pre-access testing. Management was notified and the individual files were j

annotated to reflect the positive results and management's actions.

i Management was notified of the one licensee employee that tested positive during random testing. The employee was denied unescorted access and suspended for fourteen days pending recommendations from the Employee Assistance Program evaluation. Upon return to work, the employee signed a re-entry agreement, submitted to a re-entry test, and was granted unescorted access to the protected area. The employee was enrolled in a one-year follow-up program.

Four fcilcw-up tests were administered to the licensee employee during this reporting j

There were no reportable violations of the Fitness for Duty Program (FFD) during this

reporting period.

i PROGRAM EVALUATIONIAUDIT:

Five incidents occurred in the area of rFD and documented in the site corrective action program. The incidents involved the following issues: the non-use of the FFD testing facility visitor's log, a staff member's collection of a sample from a close personal friend; disposal of a blind sample prior to confirmatory testing; a break-down in the chain-of-custody for two samples; and the perceived degrading trend in the FFD Department l

due to the aforementioned incidents. Investigation into these incidents showed human l

performance as the root cause. No programmatic problems were identified. Corrective actions were handled through Entergy's Human Resource procedures. A NRC Inspector reviewed these condition reports and found the corrective actions adequate.
