Dear Mr. Kingsley:

This refers to the inspection conducted May 29 through July 16,1998, at the Quad Cities Nuclear

Power Station. The purpose of the inspection was to determine whether activities authorized by

the license were conducted safely and in accordance with NRC requirements. The NRC

conducted routine resident inspections in this period. Based on the inspectors' findings, overall

conduct of operations at the Quad Cities station was safe. At the conclusion of the inspection,

the findings were discussed with those members of your staff identified in the enclosed ryort.

During the period, operators were challenged by a large number of equipment failures. Although

operators responded appropriately in most cases, these failures diverted operator attention and

in some cases caused transients or entry into Technical Specification limiting conditions for

operation. A conceriod effort to correct material condition discrepancies should continue to be a

priority at Quad Cities.- Further, a number of lower level human performance errors in the

application of processes at Quad Cities occurred during this period and are worthy of

management attention.

Based on these and other findings of this inspection, the NRC has determined that four violations

of NRC requirements occurred. The violations are cited in the enclosed Notice of Violation

(Notice) and the circumstances surrounding them are described in detail in the subject inspection

report;

The first violation was of concem because operators failed to follow surveillance procedures for

operating an emergency diesel generator, despite measures put into place specifically to prevent

/.

such errors. The second violation was of concem because relays associated with the

emergency diesel generators were not adequately evaluated for operation in adverse operating

/

environments. The third violation involved a failure to follow station procedures for setpoint

changes to safety equipment. The fourth violation was related to the failure to evaluate a change

in calculated minimum voltages for safety equipment described in the Updated Fina! Safety

Analysis Report.

You are required to respond to this letter and should follow the instructions specified in the

enclosed Notice when preparing your response. The NRC will use your response, in part, to

determine whether further enforcement action is necessary to ensure compliance with regulatory

' requirements.
