CLOSEOUT OF NRC COOPERATIVE AGREEMENT NO. NRC-28-83-611 FOR RADIATION MONITORING PROGRAM The period of performance of the subject Agreement expired on June 30,1998. In accordance with OMB Circular A102, " Grants and Cooperative Agreements with State and Local Govemments," and the terms of the Agreement we require the following items to close this Agreement.

(Financial Status Report SF 269A (Short Form) enclosed VRequest/ invoice for final payment if you have already fumished the above information, please state when and to whom it was submitted. Also, infcate your point of contact by name and provide a telephone number for any future inquiries. Please submit the items requested to the address below within 30 days from the date of this letter.
