SUBJECT: NRC SPECIAL INSPECTION REPORT 50-285/98-12
Dear Mr. Gambhir:
From June 1-5,1998, a special inspection was conducted onsite at your Fort Calhoun Station
reactor facility.as a result of a turbine-driven auxiliary feedwater pump overspeed occurrence on
May 27,1998. Additional in-office inspection was performed during the June through
August 1998 time frame. The enclosed report presents the scope and results of this inspection.
One noncited violation involving two examples of inadequate control of a turbine-driven auxiliary
feedwater pump speed controller refurbishment and modification was identified. Whilo the
immediate safety implications of this violation are not significant, the programmatic implications
are of concern because they illustrate poor support of maintenance by system engineering and
the performance of safety-related maintenance without proper guidance to the craft. Both your

and our reviews of the turbine overspeed occurrence identified poor work control practices as a
primary contributor to the event. As has already been discussed, a management meeting will
be scheduled to further discuss these issues and your corrective actions.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and its
enclosure will be placed in the NRC Public Document Room (PDR).
