Scalice Chief Nuclear Officer and Executive Vice President 6A Lookout Place 1101 Market Street l Chattanooga, TN 37402-2801 SUBJECT: NRC INTEGRATED INSPECTION REPORT NO. 50-327/98-07 AND 50-328/98-07 AND NOTICE OF VIOLATION ' Dear Mr. Scalice: This refers to the inspection conducted on June 6 - July 18,1998, at the Sequoyah facility. The enclosed report presents the results of this inspection. During the inspection period, your conduct of activities at the Sequoyah . facility was generally characterized by good operations. Shift turnovers and mid-shift briefings continued to be effective. Management oversight of the problem evaluation reporting (PER) process was also noted to be, effective. Maintenance review of issues was effective with a positive finding identified fo'r_ the thorough review of the turbine driven auxiliary feedwater pump packing issue. Engineering. support was considered to be satisfactory during the period. Plant support was considered to be good.with several strengths identified in the radiation protection area and several positive findings . identified in the fire protection area. Based on the results of this inspection. the NRC has determined that
. violations of NRC requirements occurred. The violations are cited in the
enclosed Notice of Violation (NOV) and.the circumstances surrounding them are i k .. described in detail in the subject inspection report. The first violation involved failures to take prompt corrective actions for adverse conditions I related to the pressurizer level transmitters. The second violation involved 2 examples of invalid calibrations performed on the Unit 2 pressurizer level l instrument 2-LT-68-320. The NRC is concerned about these issues because ~ L inappropriate processes were used to address the degraded conditions of the l transmitter, which resulted .in a failure to inform senior site management of I L- the adverse conditions. You are required to respond to this letter and should follow the instructions specified in the enclosed Notice when preparing your response.
