The subject contract expired on November 2,1994, and we are initiating closecut action. Please Please submit your expiration / completion invoice for the final billing period under the contract for provisional reimbursement, as provided for in the contract, in an original and three copies to:

Also, please complete the following forms / reports / documents if applicable to, or required by, your contract. If negative reports are appropriate, you should provide statements to that effect.
These items should be retumed to the above address within fifteen (15) days of the date of this letter.
