SUBJECT: NRC INTEGRATED INSPECTION REPORT 50-352/98-04,50-353/98-04

AND NOTICE OF VIOLATION

Dear Mr. Rainey:

This refers to the inspection conducted on May 19,1998, through July 6,1998, at your

Limerick Units 1 and 2 reactor facilities. The enclosed report presents the results of this

inspection.

During the 7-week inspection period covered by this report, the conduct of activities at

Limerick Units 1 and 2 was generally characterized by safe and conservative operations

and decision making. Your operators maintained both units at essentially 100% power

following their respective unit start-ups from their outages.

The inspectors reviewed several incidents during this inspection period that challenged

your control room operators and staff. These included, an inadvertent standby liquid

control system actuation, both subsystems of the reactor protection system being

inoperable concurrently, and main steam bypass valve operation during the Unit 1 reactor

start-up. Although your operator's response was acceptable, all of these instances

involved areas for procedure improvement. Further, during the overhaul of the D14

emergency diesel generator, several procedure adherence deficiencies were noted cross-

departmentally. Examples of procedure adherence deficiencies noted in this inspection

report and in prior reports indicate declining performance in this area.

Based on the results of this inspection, the NRC has determined that violations of NRC

requirements occurred. These violations are cited in the enclosed Notice of Violation

(Notice) and the circumstances surrounding them are described in detail in the subject

inspection report. The first violation is of concern because it involves inadequate testing

for which your corrective actions were less than adequate. The second violation is of

concern since the missed report appears to have occurred as a result of an inadequate

process. You are required to respond to this letter and should follow the instructions

specified in the enclosed Notice when preparing your response. Tne NRC will use your

response, in part, to determine whether further enforcement action is necessary to ensure

compliance with regulatory requirements.
