Attachment I' Response to Appendix A - Item A Violation As a result of the inspection conducted on August 31, 1987 to October 11, 1987, and in accordance with 10 CFR 2, Appendix C (Enforcement Policy 1987),
the following violation was identified. ,
l 10 CFR 50, Appendix B Criterion X and the Limerick Preliminary Safety Analysis Report Appendix D.4.6 and D.5.3.2 require that a program for inspection of activities af fecting quality be established and executed by or for the organization performing the activity to verify conformance with the documented instructions, procedures and drawings for accomplishing the activity. ,
Contrary to the above, on August 31, 1987, wiring within the Power Generation Control Complex was found in configurations not specified in connection diagrams 865E214TR and 807E560TR.
Specifically within panel P-618:
At device EJ a jumper from TBI-2 to TBI-8 is missing.
- At device EJ TBI-8 wire EJ-8/DE-9 is improperly landed.
At device CV TB10-4 wire EEE8-9 is labeled as EEE-9.
At device EF TB4-4 wire JCD-1 is labeled as JCD-2.
At device EH TB2-4 wire GGG1-12 is labeled as GGG1-2.
And within panel P640:
At device EG TB1-2 wire F16-1 is labeled as F15-1.
The factory wiring had been inspected in accordance with the General Electric Nuclear Engineering Business Office quality program.
This is a Severity Level IV Violation (Supplement II).
Response
1 As a result of the violation, an overall wiring inspection is being performed on all the PGCC panels wired at the GE NEB 0 Panel Shop in San Jose.
This inspection is in progress and is expected to be completed by 12/31/87.
The results of the reinspection are reviewed by GE Engineering personnel, and Field Deviation Disposition Requests (FDDR) are prepared to identify the correct configuration for drawing corrections or for wire re-labeling or other appropriate rework. The FDDR's are being issued as the inspection is completed and neces.sary re-labeling or rework is in progress and will be !
completed early in 1988.

- The mis-labeling occured at the f actory during the late 1970's and the-GE panel shop at San Jose has now been disbanded. Corrective action to prevent a recurrence at this location is no longer feasible.
Af ter tt . inspection and associated FDDR's are issued a final review of i- the conditions will be performed to determine if any further corrective action is warranted. We expect this analysis to be. complete by 6/1/88.
