Attachment to VM 87-0324 Page 1 of 2 December 1, 1987 Violation (482/8722-01): Pailure to Perform Surveillance in Accordance with Procedures Pinding Technical Specification (TS) 6.8.1 requires that,

" Written procedures shall be established, implemented, and maintained covering...a..

The. applicable procedures recommended in. Appendix A of RG 1 33, Revision 2,

.Pebruary 1978." Section 8 of Appendix A of RG 1 33 states that activities.related to surveillance tests should be covered by written procedures.

Administrative Procedure ADM 02-300, Revision 8, " Surveillance Testing," and j

Surveillance Test STS PP-602, Revision 5, '" Electric Motor Driven Pire Pump.

1PP01PA - Monthly Operation" had been established and ' implemented in accordance with the above TS.

Sections 5.2 and 5.3 of STS PP-602 required that'the electric mot'or driven fire pump be started using the switch on the local panel during the monthly

tests for January, March, May, July, September, and November.

Daring the alternate months the procedure required that the pump should be started from the control room.

Contrary to the above, during'the' performance of Surveillance. Test STS PP--

602, Revision 5,

" Electric Motor Driven Fire Pump 1PP01PA

~ Monthly Operation,

" on April 1, 1987, the pump was started from the control room and was not started again from the control room during the performance of the surveillance test until August 1, 1987.

Reason Por Violation:

STS PP-602 had been performed on May 1, 1987, which' started the electric motor driven fire pump from the local panel as required by sections 5 2 ' and 5.3 of the surveillance procedure.

The monthly test was next scheduled bo be performed on June 1,

1987, which would have started the pump from the control room. On May 30, 1987, control room. personnel decided it was a good time to perform STS PP-602 instead of waiting until, June 1, 1987, when it was scheduled. They subsequently performed the surveillance, tfollowing the procedural requirements to start the pump locally.in the ' month of: May.

surveillance was next performed on July 1, 1987, which again required starting the pump locally.

i The reason for this violation is a failure of control room personnel-to recognize that performance of the surveillance two days early would result' q
