Survey and draft letter of transmittal Rosetta, l

Per our discussion of yesterday aftemoon, I have attached copies of the survey developed by the risk review working group and a draft All Agreement State Letter for transmitting that survey to the States. I have made the changes discussed with you and Paul Lohaus, but the location of the OMB Approval statement in the transmittal letter may need to be changed.

The working group realizes that the thickness of the survey may elicit an immediate negative response, but we believe that the group of individuals who are being asked to respond must be heard from in some detailif the risk review is to be successful. The working group has designed, tested, and modified the survey to be easy to complete. The large majority of the responses are simple X marks and quantity indications. In addition, the survey had to be designed to tap the knowledge of the full spectrum of prospective respondents. but few of the individual respondents are expected to be familiar with every system about which the survey has questions. Thus, consistent with the instructions provided, individual respondents are not expected to answer every question in full. The survey provides individcls with the opportunity to indicate that they are not familiar with a particular system and to go on quickly. In some cases, an indication of lack of familiarity with a system will allow an individual to skip a whole block of questions. The working group specifically does not want individuals to tum the eurvey into an individual or i office research project or leaming exercise. Thus, we've asked that responses come only from between l the respondents ears.

Based on the performance and feedback from our test respondents, we believe that individuals responding to the survey will find the questions dealing tvith systems about which they have knowledge easy to answer and the " don't know/not familiar" options easy to understand.
