TOUR OF SHOREHAM FACILITY HOUSEKEEPING - Numerous inctances of poor housekeeping were f ound during the walkthrough. These included stray tools, rags, nails, excess scaffolding, and other materials left astr ay f ollowing L completion of work activities. What actions will be taken to improve post maintenance cleanup of work ar eas?

GREASING OF - Improper lubrication practices at other plants has led to ROTATING f ai l ur et., of rotating equipment. Spec i f i call y , this is a result  ;

MACHINERY of not removing the drain plugs prior to greasing which causes '

the grease to cor e out along the shaft where it is then sucked into the motor. Several pumps inspected did not have drai-n plugs which wer e easily accessi b l e and some showed i ndi c a ti ons of over greasing. Attention should be directed to ensuring  ;

drain plugs are easily accessiale and personnel ar e properly  !

trained. What changes will be implemented to prevent po ten ti al overgreasing of safety related pumps?

MISSING PIPE-- Numerous instances of open anded piping were found on high CAPS energy, or contaminated systems. All were isolable by at least one valve but attention should be di rect.ed to evaluating if caps should be installed. This is a personne] cafety and r adi ol ogi c al issue. What actions will be t a l. e n to address this issue?
