DUANE ARNOLD INSPECTION REPORT 50-331/98008(DRP) AND

- NOTICE OF VIOLATION.

Dear Mr. Franz:

On June 9,1998, the NRC completed a routine safety inspection at your Duane Amold Energy

L

Center (DAEC) facility. The enclosed report presents the results of that inspection.

..

Based upon the findings during this 6-week period, overall performance was characterized by

safety conscious operations. During this inspection period, improved performance was noted

from the first half of the scheduled refueling outage. The number of self-revealing events, human

i

errors, communication problems, and planning and scheduling deficiencies, as detailed in

inspection Report 50-331/98004, decreased. However, we are concemed about three violations

!

of NRC requirements that were identified. The first violation is of concem because of the

importance of maintaining your drywell free of trash and tools following outage activities. The

i

second violation involved inadequate post-maintenance testing. The third violation involved the

failure to report the condition of the high pressure coolant injection (HPCI) system room

compartment as being outside of ihe design basis due to a postulated high energy line break.

While your staff was proactive in broadening the scope of the evaluation to the HPCI room walls,

NRC intervention was necessary to ensure that this issue was reported as required.

These violations are cited in the enclosed Notice of Violation (Notice), and the circumstances

surrounding the violations are described in detail in the enclosed report. Please note that you are

i

i

required to respond to this letter and should follow the instructions specified in the enclosed

Notice when preparing your response. The NRC will use your response, in part, to determine

whether further enforcement action is necessary to ensure compliance with regulatory

requirements.

~ We are also concemed about the three mispositioned valve events that occurred in an 11-day

period during the refueling outage. Two of the mispositioned valve events resulted in two non-

cited violations. The third mispositioned valve event was found by your staff during the conduct

of a surveillance test and did not result in a violation.
