NRC INSPECTION REPORT NO. 50-331/98010(DRS) AND NOTICE OF

VIOLATION

Dear Mr. Franz:

On May 5 and 6,1998, the NRC conducted an inspection at your Duane Amold Energy

Center. The results of the inspection were communicated to your management on June 4,

1998, during a telephone conference. The enclosed report presents the results of this

inspection.

The inspection involved an examination of selected electrical maintenance activities. The

inspection consisted of a selective examination of procedures and representative records,

observations, and interviews with personnel. Specifically, the inspection focussed on

activitics associated with electrical Grayboot applications and the training and qualification

process to perform these applications.

Based on the results of this inspection, two violations of NRC requirements were identified.

The first violation concemed the failure to follow procedural requirements during

performance of maintenance activities. The second violation concemed failure to establish

a program which will provide for indoctrination and training of maintenance personnel

performing Grayboot application activities to assure that suitable proficiency is achieved and

maintained prior to performing these specialized applications. Our identification of the

installation of an undersized Grayboot Kit and your identification of a Grayboot connector

failure due to an improper crimp, underscores the need for adequate training in this area.

The violations are cited in the enclosed Notice of Violation (Notice), and the circumstances

surrounding them are described in detailin the enclosed report. Please note that you are

required to respond to this letter and should follow the instrttions specified in the enclosed

Notice when preparing your response. The NRC will use your response, in part, to

determine whether further enforcement action is necessary to ensure ccmpliance with

regulatory requirements.
