1RIDIUM-192l RADIOGRAPHY SOURCE On March 14, 1975, Office of Inspection and Enforcement was informed by the Office of Nuclear Materials Safety and Safeguards that it had received information from the State of Louisiana on March 13, 1975, that a 27 curie Iridium-192 radiography source in its shielded container was lost in transit from Gama Industries, Baton Rouge; Louisiana to Stone and Webster Engineering Company c/o Duquesne Light Company Beaver j
Valley site, Pittsburgh, Pennsylvania.
The source was shipped in a gg DOT-approved container on February 25, 1975, and arrived in New York City on the 26th.
Stone and Webster did not report the source missing until March 12, 1975.
The radiation levels with the source in the

shicided container are 20 mr/hr at the surface and 1.5 mr/hr at 1 meter.
Office of Inspection and Enforcement, Region I is determining whether a proper search for the source is being conducted.
This matter vill be followed closely.
State and appropriate federal agencies have been notified. There is no known news media awareness of this matter.
