- Licensee: - National Tank Company, Tulsa, Oklahoma

Possible Overexposure (Type B) l Licensee notified Region IV by telephone on 6/27/75 that a radiographer
-l potentially received a whole body exposure of 12 rem and as yet an undetermined hand exposure while performing radiography at the licensee's shop on the morning of 6/27/75.
The radiographer, using a camera containing 40 C1 of Ir-192, failed to make surveys to ensure the source was in the shielded position'and performed
- work for approximately 2-1/2 minutes around the exposed source.
N Licensee evaluation indicates a 12, rem whole body exposure.. Extremity dose being evaluated.
Film badge air mailed to U. S. Testing for

prompt processing.
Radiographer taken to Class-Nelson Clinic, Tulsa for examination.
No publicity and action by Region IV to be determined after film badge processed.
OIE (Region IV) received notification of this occurrence by telephone from licensee's Radiation Safety Officer at 11:45 a.m. CDT on 6/27/75.
