Commonwealth Edison Company Docket No. 50-354-c<

l As a result of the inspection conducted on September 22 through November 5, 1987,'and in accordance with 10 CFR Part 2, Appendix C - General Statement of Poli,cy.and Procedure for NRC Enforcement Actions (1985), the following violation was,1dentified:

10 CFR 50, Appendix B, Criterion V, as implemented by the Commonwealth Edison I

Operational QA program, requires that activities affecting quality be l

prescribed by documented instructions procedures, or drawings, of a type l

appropriate to the circumstances, and be accomplished in accordance with these peg. instructions, procedures, or drawings.

  • ' Con: rad to"the above, Proctiure No. 00P 6500-4, which is used in racking-out 4160 volt ranua11y operated air circuit breakers, was inadequate because of the following:

1.

Theprecautionstatementint$eprocedurewhichstatedthatopeningthe racking' screw shutter on a closed circuit breaker would trip the breaker was incorrect.

2.

The procedure did not caution the operhor to verify the position of the open/close indicator prior to proceeding to rack-out the breaker.

3.

The procedure did not caution the operator to actuate the manual trip button prior to rack-out of the breaker.

This is a Severity Level IV violation (supplement I).
