In accordance with 10CFR55.23 and 10CFR55.33, Southrn Nuclear Operating Company (SNC) is submitting a Certification of Medical Examinat.'on form (NRC Form 396) for license !! SOP 20459-2, docket #55-30197. A recent eye examination performed on.fuly 9, 1998, indicates that this individual now requires correctise lenses when performing licensed duties.
SNC requests that the individual's license be amended to reflect this change in status.
