NRC INSPECTION REPORT 50-277/98-06,50-278/98-06;
- Dear _ M6 Rainey:
- On June 22,1998,the NRC completed an inspection at the Peach Bottom Atomic Power
Station.JThe enclosed report presents the results of that inspection.
- During the seven-week ' period covered by this inspection period, your staff continued to
,
'
operate both units at the Peach Bottom Atomic Power Station safely. The decision by your
staff to proceed with on-line scram solenoid pilot valve (SSPV) replacements reflected a
strong safety perspective and was prudent, based on the degraded conditions noted in
'
laboratory analyses of the SSPV parts. We also considered the SSPV maintenance

activities to be particularly well-executed. We observed that Nuclear Maintenance Division
,
- technicians and operations personnel displayed good procedure usage and sound work
l practices.- Supervisory personnel also provided very good coordination and oversight.

Based on the'results of this inspection, the NRC has determined that four violations of NRC
requirements occurred. ' These violations are cited in the enclosed Notice of Violation
,
'(Notice) and the circumstances surrounding them are described in detail in the subject

- inspection report.
The first violation resulted from the trip relay for the Main Control Room Emergency
- Ventilation (MCREV) radiation monitor not being in the tripped status for approximately
E 28 hours3.240741e-4 days <br />0.00778 hours <br />4.62963e-5 weeks <br />1.0654e-5 months <br /> while the 'B' channel radiation monitor was inoperable. Operations personnel ~
failed to perform a positive verification that the relay tripped when a jumper was installed.
?The corrective actions from a similar event on July 10,1997, failed to prevent this event.
The second violation involved the failure of site engineering personnel to take prompt and
' effective corrective actions to change several procedures for the residual heat removal
{
system which contained errors that were identified by Quality Assurance personnel. The
failure to revise the procedures resulted in a residual heat removal system valve being left
'
incorrectly positioned when plant operators used an unrevised procedure on March 4,
1998. We are concerned'with the number of valve mispositioning events that continued to
occur during this inspection period. Please address any generic implications to other valve
- mispositioning events and any comprehensive corrective actions for plant configuration
control issues in your response to this violation.

The third violation resulted from Unit 3 recirculation flows being mismatched for over
12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> following the runback of the 3A recirculation pump on June 7,1998 during an
electrical storm. This mismatched flow resulted in a violation of Technical Specification 3.4.1. We are concerned that during this event, operations and engineering personnel did
not understand that the mismatch in recirculation flows caused excessive vibration
stresses on the jet pumps and jet pump riser braces. They also failed to recognize that
Unit 3 was in single loop operation following the runback of the 3A recirculation pump.
The fourth violation occurred from the failure of operations personnel to verify that the
difference between the temperature of the reactor coolant in the recirculation loop to
be started and the coolant in the reactor pressure vessel was :s;50 F within 15 minutes
prior to each startup of a recirculation pump. The required technical specification
surveillance verification was not performed during six of the first recirculation pump starts
which occurred on both units from June 25,1996 until March 23,1998. In addition, on
October 29,1997, the Unit 3 temperature differential was 84*F within 15 minutes of the
recirculation pump start.
You are required to respor.d to this letter and should follow the instructions specified in the
enclosed Notice when preparing your response. The NRC will use your response, in part,
to determine whether further enforcement action is necessary to ensure compliance with
regulatory requirements.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter
and its enclosures will be placed in the NRC Public Document Room (PDR).
