We recently identified that due to an administrative oversight, Form NIS-2, " Owner's Report for Repairs or Replacements" had not been enclosed with our previous refueling outage summary reports.
Copies of NIS-2 Forms are enclosed in compliance with ASME Section XI, Article IWA-6220.
A copy of this letter with enclosure is being submitted to the State of New Hampshire.
