A routine, unannounced inspection was conducted by Ms. Jean A. Gresick-Cioffi

of this office on October 5-9, 1987, to review certain elements of your radiation protection program.

Our insoector also reviewed your evaluation of the failed manufacturing equipment and subsequent intakes of radioactive materials by eleven Pellet Shop workers on August 12, 1987. As a result of this review and an allegation received during the inspection, the inspection was extended through October 19-23, 1987 to further assess the status of radiological controls at this facility.

As a result of this inspection, our inspector identified significant deficiencies in the radiation protection program.

Several of your activities I

were not conducted in full compliance with regulatory requirements.

Apparent violations are discussed in the enclosed report.

Based upon discussions between Dr. Ronald Bellamy of this office and yourself, in a telephone conversation, on October 26, 1987, a Confirmatory Action Letter l

was issued on October 27, 1987. This letter documented your immediate corrective action commitments in two areas of degraded radiological controls at the manufacturing facility. Also discussed in the telephone conversation was the need to schedule an Enforcement Conference in the Nuclear Regulatory Commission Region I office to discuss these matters with you. An Enforcement Conference has been scheduled for December 1, 1987 at 11:00 a.m.
