United States Nuclear Regulatory Commission Region Ill 801 Warrenville Road Lisle, Illinois 60532-4351 Portsmouth Gaseous Diffusion Plant (PORTS)- Docket No.70-7002 - 10CFR95.25 - Event Report 98-11 In accordance with 10 CFR 95.25 (i), this report summarizes the actions taken as a result of the discovery of an unattended security container found opened. Enclosure I lists the actions taken.

On June 15,1998, at approximately 0125 hours0.00145 days <br />0.0347 hours <br />2.066799e-4 weeks <br />4.75625e-5 months <br />, guard force personnel making routine security checks of classified containers discovered container #43-50012 unattended and unlocked. The container housed classified matter and was located in office #342, in the Customer Order Management Section of the X-710 Laboratory. The Plant Shift Superintendent (PSS) was notified at approximately 0137 hours0.00159 days <br />0.0381 hours <br />2.265212e-4 weeks <br />5.21285e-5 months <br /> and the container was secured by guard force personnel. At 0230 hours0.00266 days <br />0.0639 hours <br />3.80291e-4 weeks <br />8.7515e-5 months <br />, an Order Planner (OP), the individual who last accessed the container on June 14,1998, was notified at her residence that the container was found unattended and unlocked.

i On June 15,1998, at approximately 0630 hours0.00729 days <br />0.175 hours <br />0.00104 weeks <br />2.39715e-4 months <br />, the OP reported to work and conducted an inventory of the container and found none of the classified documents or items disturbed or missing.

On June 15,1998, a security investigator conducted interviews with the OP. The OP said that she had accessed and closed the container the previous day. The subject stated after closing the container she checked each drawer but failed to clear the combination by spinning the dial and pulling the locking handle prior to exiting the office. A review of the SF-702 " Security Container Check Sheet," dated June 14,1998, confirmed that the OP opened the container at 1800 hours0.0208 days <br />0.5 hours <br />0.00298 weeks <br />6.849e-4 months <br />, and closed and checked the container at 1830 hours0.0212 days <br />0.508 hours <br />0.00303 weeks <br />6.96315e-4 months <br />.

A review of Procedure XP2-SS-SS1039 " Handling and Control of Classified Documents," describes the action steps that are to be taken when opening, closing and checking a classified container. The procedure states that upon opening a classified repository the custodian shall initial the SF-702 i

l assigned to the repository and record the date and time of access in the " opened by" portion of the g

SF-702. The procedure further states, when securing a classified repository close all drawers, spin

the combination dial at least four times in either a clockwise or counterclockwise direction, tests each drawer to assure that the locking mechanism is operable and initial and enter the time the l

Following the securing of the repository the procedure states, if another cleared individual is readily available, request the individual conduct and document a double check of the container. Spin the combination dial at least four times in either a clockwise or counterclockwise direction, test each drawer ensuring the locking mechanism is engaged and record his/her initials and time in the " checked by" column of the SF-702.

If another cleared individual is not readily available, the double check may be conducted by the individual securing the container.

The OP did not perform all of the steps in procedure XP2-SS-SS1039 that would have ensured the container was properly secured. On June 14,1998, at 1830 hours0.0212 days <br />0.508 hours <br />0.00303 weeks <br />6.96315e-4 months <br />, the OP was alone and another individual was not readily available to double check the container. The OP conducted the double check and initialed the appropriate blocks on the SF-702. The OP failed to clear the combination by spinning the dial and pulling the locking handle.

At PORTS, an unlocked security container found unattended constitutes a security infraction.

Security containers are used to protect classified information. The unlocked and unattended security container was within the plantsite controlled access area. Personnel having access to the controlled area must have the appropriate access authorization level for entry or be under positive escort of a cleared individual. Due to these facts and since an inventory of the container found all contents to be in order, it is believed that no unauthorized access was gained to the container and no classified information or material was compromised.

On June 15,1998, the OP was issued a security infraction. The OP also reviewed the section of XP2-SS-SS1039, which specifies the custodial responsibilities for the protection of classified information. The OP was provided remedial training on the correct process for j

securing a security container.
