SEQUOYAH NUCLEAR PLANT (SQN) - INADEQUATE DESIGN OF DIESEL GENERATOR (D/G) j BUILDING SPRINKLER AND STANDPIPE SYSTEM t In TVA's SQN Fire Protection Program Reevaluation, which was transmitted to i NRC on January 24, 1977, TVA stated, "A single impairment in the fire )'

protection system or direct support systems shall not impair both primary and backup fire protection capability." Where automatic fire suppression systems are required and provided, they are considered the primary fire protection l capability; fire hose stations are the backup fire protection. In the D/G J Building, both the automatic sprinkler system for the corridor and the corridor fire hose Plation are supplied by a single and Common line from the l fire protection yard water main. Also, both systems are controlled by a l common post indicator valve. This deficiency was identified at SQN after a I l

similar deficiency was identified by NRC at Watts Bar Nuclear Plant in 1982.

A design change request was initiated in October 1982 by SQN to provide a second supply line for the D/G Building corridor. The following compensatory measures were initiated and will be maintained until implementation of the design change. A wheeled 150-pound dry chemical fire extinguisher is provided in the D/G Building corridor. It is routinely inspected and maintained. This unit has significant fire fighting capability and is adequate to control any fire in the corridor area. Additionally, a portable foam generating unit  !

containing 32 gallons of 3-percent concentrate fire-fighting foam is stored in l the main entry foyer of the D/G Building. This unit is in the normal travel l path of the fire brigade in the event of a response to a fire in the corridor j area. Sufficient fire hose is maintained in the fire equipment house at the D/G Building to supply water to this unit. The foam hose and stream reach are capable of reaching all areas of the corridor.

The second supply line has not yet been installed. SQN is presently developing the design and implementation plan for the removal of CO2 systems I

and the installation of automatic sprinkler fire suppression systems at the l D/G Building. The installation of the second water supply line will be j included as part of this work and is scheduled for completion six months after the unit 2 cycle 3 refueling outage.
