SUBJECTas MOTIER'0F EXPIRATION Your NRC license specified above authorizing use of nuclear-powered pace-makers will expire on the date shown. We want to call your attention to somo special conditions on your license and explain how these conditions effect your decision whether to renew the license.
In the United States, anyonePatients who possesses and uses a nuclear pacemaker must in whom you have implanted pacemakers be covered by a license.
are covered by the hospital"s license; see Condition 11 of your license.
In Conditions 15 and 16 of your license, you are commited to following all patients with implanted pacemakers for the life of the patient or until the pacemaker is removed and returned to the manufacturer for proper disposal.
Follow-up procedures must be in accordance with the protocol established by the manufacturer and approved by HRC. Termination, renewal, and fee infor-motion is included in the enclosure.
