OPERATING REACTORS EVENTS BRIEFING JULY 8,1998 - BRIEFING 98-06

On July 8,1998, we conducted an Operating Reactors Events Briefing (98-06) to inform senior -

managers from the Offices of the Commission, EDO, AEOD, NRR and regional offices of selected events that occurred since our last briefing on July 1,1998. Attachment i lists the attendees.' Attachment 2 presents the significant elements of the discussed event.

The statements contained in the attached briefing slides represent the best information currently available to the NRC. Future followup could produce new information that may alter the NRC's current view of the event discussed.

JUNE 26,1998 0200 - LICEN'SEE DOWNGRADED TO AN UNUSUAL EVENT BASED ON RESTORATION OF ONE OFFSITE POWER '

1358 - LICENSEE EXITED UNUSUAL EVENT BASED ON RESTORATION OF SECOND OFFSITE POWER SOURCE.

EVALUATION I

O THREE 345-KV LINES SUPPLY OFFSITE POWER. l TWO LINES (OHIO EDISON AND LEMOYNE)

SUFFERED NO DAMAGE FROM STORM. THIRD LINE (BAYSHORE) HAD DAMAGE TO 11 TOWERS.

IN SWITCHYARD, DAMAGE TO NUMEROUS INSULATORS AND HIGH VOLTAGE CONNECTORS.

PART OF TURBINE BUILDING ROOF WAS BLOWN OFF.

l ALL NORMAL AND EMERGENCY TELE-COMMUNICATIONS WERE LOST. CORPORATE I MICROWAVE SYSTEM AVAILABLE.

O EDG #1 FAILED TO START FROM CONTROL ROOM.

STARTED LOCALLY. CAUSE WAS DEFECTIVE

. DAVIS-BESSE 98-06 9 EDG #1 DECLARED INOPERABLE DUE TO ELEVATED ROOM TEMPERATURES ABOVE 120 DEGREE LIMIT. CAUSE WAS FAILED DAMPER HYDRAULIC ACTUATOR, CAUSING DAMPER TO Fall OPEN. DAMPER WAS MANUALLY CLOSED AND FANS PROVIDED TO COOL ROOM. EDG #1 REMAINED FUNCTIONAL THROUGHOUT EVENT.

l 9 HIGH TEMPERATURE IN EDG #2 ROOM ALSO (ITS DAMPER ACTUATOR ALSO FAILED) BUT 4 REMAINED BELOW 120 DEGREE LIMIT.

G ONE MS SAFETY VALVE LIFTED BELOW SETPOINT AND DID NOT FULLY RESEAT.

HEADER PRESSURE REDUCED TO SHUT VALVE.

G ATMOSPHERIC VENT VALVE #2 FAILED TO CLOSE AUTOMATICALLY DUE TO FAILURE OF 1/P CONVERTER. VALVE FAILED AT 10% - 14%

OPEN POSITION WITH 0% DEMAND. VALVE WAS OPERATED MANUALLY.

DAVIS-BESSE 98-06 e WHEN RETURNING TO OFFSITE POWER, CIRCUlT BREAKER ABDC1 FAILED TO CLOSE WHEN PARALLELING EDG #1 TO OFFSITE POWER. CAUSE WAS INADEQUATE MECHANICAL CLEARANCE BETWEEN THE TRIPPING TRIGGER CAM AND TRIP PLUNGER PIN.

O WHEN RETURNING TO OFFSITE POWER FROM EDG #2, RECEIVED EDG BREAKER TRIP ON REVERSE POWER. POSTULATED THAT FAILURE OF R3 RELAY CAUSED FAILURE OF BYPASS FOR REVERSE POWER TRIP FUNCTION ON LOOP.

CORRECTIVE ACTIONS O BAYSHORE TOWERS BEING REPAIRED

(~2 WEEKS).

DAVIS-BESSE 98-06 G TEMPORARY ROOF INSTALLED ON TURBINE BUILDING.

O EDG #1 SWITCH CONTACT BLOCK REPLACED.

O DAMPER HYDROMOTORS REPAIRED. ROOT CAUSE EVALUATION TO BE PERFORMED.

G 1/P CONVERTER REPLACED FOR ATMOSPHERIC VENT VALVE #2. ROOT CAUSE EVALUATION TO BE PERFORMED.

O BREAKER ABDC1 REPLACED WITH SPARE. ROOT CAUSE TO BE DETERMINED.

.O R3 RELAY HAS BEEN REPLACED AND SUCCESSFULLY TESTED.

I PLANT REMAINED IN HOT STANDBY IN ANTICIPATION OF RETURNING TO POWER.

AGENCY RESPONSE O REGION: INCIDENT RESPONSE CENTER ACTIVATED HOUR MONITORING.

DAVIS-BESSE 98-06 9 13-MEMBER SPECIAL INSPECTION TEAM DISPATCHED.

DETERMINED LICENSEE RESPONSE AND CORRECTIVE ACTIONS WERE SATISFACTORY.

FINDINGS WILL BE DOCUMENTED IN INSPECTION REPORT 50-349/98012.

G RESTART PANEL FORMED ALL RESTART ISSUES RESOLVED ON JUNE 29, 1998.

MC 0350 PROCESS USED TO DETERMINE RESTART CAPABILITY.
