SUl#tARY OF AUDIT ON SEPTEMBER.8-10, 1987 -
CABLE TRAY HANGER AS-BUILTING On September 8-10,_1987, the NRC staff and its consultants conducted an audit of the as-built program for cable tray hangers at.the CPSES site. The cable tray hanger as-built program is being performed by.Ebasco Services (CAP)porated (Ebasco) as part'of the TV Electric Corrective Action Program Incor for design validation. A list of persons involved with the audit is provided in Enclosure 1.
The purposi of the audit was to determine.the adequacy of the as-builting activities for.the CPSES seismic Category I cable trays and supports.
A flow chart of the as-builting activities is shown in Figure.1 of this sunmary.
The scope of the audit included:
a.
a review of the as-builting procedures-for technical
- adequacy, b.
a review of the implementation of procedures and process flow chart, c.
a review of the selected installed supports and tray runs for agreement with final drawings. and d.
a walkdown of cable tray runs and. supports for any visually apparent discrepancies.
The Unit 1 as-builting effort involved approximately 4500 supports of which approximately 10% required design modifications as a result of verification' analysis. Approximately 90% of the supports required Type-1 modifications which are construction deficiencies-(e.g., loose bolts, inissing washers, and gaps behind base plates) reported by the walkdown engineers.
Review of As-Builting Procedures The as-builting and design adequacy verification procedures were reviewed and discussed.with the Ebasco personnel involved with the implementation of these procedures. The procedures which were reviewed are listed in Enclosure 2.

Other procedures which are listed in Enclosure 2 were developed for obtaining

information on some s. pecific attribute which was required subsequent to perfonning the initial evaluation of the tray and/or supports.
l The audit team found that-the procedures being used in the as-builting and f
design adequacy verification program contain sufficient detail to be implemented by the responsible personnel. Walkdown engineers are formally ~ trained in the use of the procedures and exhibited familiarity with the requirements.-
i Review of Design Process and Procedure Implementation The work sequences and document flow were reviewed and discussed with walkdown engineers and their supervisors to determine if the applicable procedures were properly implemented.
Four completed Unit'1 packages were reviewed for implementation of procedure requirements.

and the documents were found to follow procedure requirements and were l
well controlled. Only completed Unit 1 packages were reviewed during this audit. The staff will continue to review completed Unit 2 packages during followup audits.
Review of Installed Supports to Design Drawings Actual walkdowns were conducted for 5 Unit 1 tray spans including 9 supports to determine conformance to drawing and procedural requirements. Similar walkdowns were performed in Unit 2 for 4 tray segments, including 5 supports l
(See Enclosure 2).
The span drawings and the support drawings were compared to the actual installations by checking the following characteristics during the walkdowns:
Configuration and location of the spans or segments and the supports Support identification l
Types of clamps Types and sizes of welds Types, sizes, and material of bolts-Member orientation Tray location on tier Anchorage configuration Attachments to tray, i.e., conduit L1andL2 distances (forUnit2only)
Tray nunbers or segment numbers The results of the walkdowns of. tray runs and-supports for Unit I revealed minor discrepancies..Walkdown of Unit 2 cable trays and supports resulted in noting an apparent excessive gap behind a base plate (CTH-2-11982) and the location of 1/2" HILTI's near one of the baseplates which do not appear on the:
support drawing. The TV Electric QA inspection report for this support (IRME-27056) was.provided to the audit team. -The team is' continuing its review of the acceptability of the excessive gap behind the baseplate. This issue will be reviewed further in a followup audit.

Audit Sumary of 9/8-10/87 Review of Visual Discrepancies A walkdown was conducted through randomly selected Unit 1 and Unit 2 areas in order to note any visually apparent discrepancies. Visually apparent tray discrepancies which were noted during the walkdowns included non-standard-1 splice plates, side wall cutouts, and non-standard hinged fittings used for small changes in the horizontal direction. The splice plate and sidewall l
discrepancies will be resolved by means of procedures which are currently in preparation. The non-standard tray fitting discrepancy is covered as part of 1
a Corrective Action Request, CAR-87-031.
A followup audit will be conducted to review the resolution of these discrepancies.
