Portsmouth Gaseous Diffusion Plant (PORTS)- Docket No.70-7002 - 10CFR95.25 - Event Report 98-10 In accordance with 10 CFR 95.25 (i), this report summarizes the actions taken as a result of the discovery of an unattended security container found opened. Enclosure 1 lists the actions taken.

On June 4,1998, at approximately 0220 hours0.00255 days <br />0.0611 hours <br />3.637566e-4 weeks <br />8.371e-5 months <br />, guard force personnel making routine security checks ofclassified containers discovered container #5549 unattended and unlocked. The container housed l

classified matter and it was located in the Site and racilities Support Organization section of the X-l 100 Administration Building. The Security Program M mager (SPM), the authorized custodian of j

l the container, was immediately notified at his residence.

On June 4,1998, at approxima:ely 0330 hours0.00382 days <br />0.0917 hours <br />5.456349e-4 weeks <br />1.25565e-4 months <br />, the SPM arrived on plantsite, At approximately 0351 l

the subject examined the contents of the' container and found none of the classified documents l

disturbed or missing.

On June 4,1998, a security investigator conducted interviews with the SPM. The SPM stated that prior to exiting the office on June 3,1998, at 2300 hcurs, he placed an unclassified document in the container, checked all the drawers and spun the combination dial one revolution. A review of the SF-702 "Secutity Container Check Sheet," dated June 3,1998, confirmed the SPM opened the l

container at 0715 hours0.00828 days <br />0.199 hours <br />0.00118 weeks <br />2.720575e-4 months <br />, closed the container at 0830 hours0.00961 days <br />0.231 hours <br />0.00137 weeks <br />3.15815e-4 months <br /> and checked the container secure at 1730 hours0.02 days <br />0.481 hours <br />0.00286 weeks <br />6.58265e-4 months <br />. There were no other written entries on the SF-702.

o A review of Precedure XP2-SS-SS 1039 adequately states the action steps that are to be taken when opening, closing and checking a classified container. The procedure states, upon opening a classified repository, the custodian shall initial the SF-732 assigned to the reposiiory and record the date and time of access in the " opened by" partion of the SF-702. The procedure further a classified repository close all drawers, spin the combination dial at least four times in either a clockwise or counterclockwise manner, tests each drawer to assure that the locking mechanism is operable and initial and enter th time the repository is secured in the " closed by" per$n of the SF-702. These procedural steps were not followed by the SPM responsible I

for securing the container.

At PORTS, an unlocked security container fcund unattended constitutes a security infraction.

Approximately 18 months ago the SPM was issued a sect.rity infraction for leaving the same container unattended and unlocked. This was the second secui ity infraction issued to this individual.

Security containers are used to protect classified information. The unlocked and unattended security container was within the plantsite controlled access area and locked behind two closed doors.

Personnel having access to the controlled area must have the appropriate access authorization level for entry or be under positive escort of a cleared individual. Due to these facts and since an inventory of the container found all contents to be in order, it is believed that no wu.uthorized access was gained to the container and no classified information or material was compromised.

On June 4,1998, the SPM was issued a second security infraction. The SPM also reviewed 1

1 the section of XP2-SS-SS1039, which specifies the custodial responsibilities for the protection of cla.csified information. The SPM was counseled on the correct process for securing a security container.

On June 4,1998, the combination to the security container was changed.

3.

On June 4,1998, a positive discipline oral reminder was issued to the SPM for his failure to secure the container.

On June 24, 1998, the classified documents in container #5549 were transferred to a classified container located in the Security Section Manager's office and container #5549 was declassified. The SPM is not a custodian of the Security Section Manager's container.
