LASALLE INSPECTION REPORT 50-373/98010(DRP); 50-374/98010(DRP) AND
Dear Mr. Kingsley:
On June 5,1598, the NRC completed an inspection at your LaSalle facility. The inspection focus
was primarily on routine operations at the station. In addition, the inspection included a review of
the status of the LaSalle Restart Action Plan actions. The enclosed report presents the results
of that inspection.
During this 6-week inspection period, overall performance at the LaSalle station was acceptable.
However, human performance errors continued to be of concem, particularly those adversely
impacting plant configuration control. For example, a failure to understand independent
verification requirements resulted in removal of the incorrect out-of-service tags and operation of
the incorrect control room ventilation system dampers. In addition, personnel began
maintenance work on the electrohydraulic control system without the planned out-of-service in
place. Your staff has made progress in the implementation of the action plan items discussed in
the LaSalle Restart Action Plan. However, human performance improvements achieved through
implementation of extensive actions in the LaSalle Restart Action Plan are not yet self-
sustaining. Continued management focus is warranted in this area.
Based on the results of this inspection, the NRC has determined that a violation of NRC
requirements occurred. This violation involved the failure to follow procedure requirements for
performing independent verification activities when non-licensed operators were returning control
room ventilation system equipment to service. Specifically, the operators did not independently
verify that the dampers they were repositioning were the correct dampers, and as a result
repositioned the wrong dampers.
You are required to respond to this letter and should follow the instructions specified in the
enclosed Notice when preparing your response. The NRC will use your respcnse, in part, to
determine whether further enforcement action is necessary to ensure compliance with regulatory
requirements.
