PRELIMINARY NOTIFICATION
, February 8, 1977 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE-PNO-77-24 This preliminary notification constitutes EARLY notice of an event of POSSIBLE safety or public interest significance. The informatica presented is as initially received without verification or evaluation and is basically all that is known by IE staff on this date.

SUBJECT:
FAILURE OF FEEDWATER SAMPLE PROBE During surveillance testing on January 21, 1977, a HPCI check valve failed to hold and allowed feedwater to flow backwards through the HPCI injection line. The HPCI system was declared inoperable and the other ECCS Systems were demonstrated operable-per Technical Specification requirements.
i The licensee was granted a Technical Specification relaxation that permitted an additional seven days of operation with the HPCI inoperable, owing in part to had weather and power demands on the system.
The facility was shutdown on February 3, 1977, for repair of the HPCI check N
valve.
The cause of the valve problem was determined to be a piece of schedule 120 stainless pipe that had become lodged in the valve. The licensee determined that a sample probe from the 24-inch feedwater line was missing and that there were ro additional loose parts in the system. The failure I
mode of the sample probe is to be analyzed including the possibility of generic implications.
The reactor was restarted on February 6, 1977.
The licensee does not plan to make a press release.
IE:IV received notification regarding the sample probe failure from the Station Superintendent, Cooper Nuclear Station, at 3:45 p.m., on February 6, 1977.
