Reporting SCDHEC NPDES Permit #SC0001848 Notice of Violation Verified 6/16/98 Attached with this letter is the response report sent to South Carolina Department of Health and Environmental Control regarJing a paperwork violation on the April 1998 Discharge Monitoring Report Correspondence. Following discussion with their representative, it can be concluded that the event was minor in nature. We have developed a comprehensive response and action plan to assure all correspondence is correct.

NPDES Permit #0001848 Response to 6/4/98 Notice of Violation l
l In response to your letter on June 4,1998, regarding submission of the monthly NPDES Discharge Monitoring Reports, we are providing the following response:
(1) The April DMR was corrected o 1 the day following receipt of the note that the concentration was missing and hand delivered to the Data Administration Section.

(2) The errors noted occurred in three different calendar years, and involved three y
separate paperwork issues. The following errors were noted:
t The initial referenced error in 11/% involved a failure to properly understano e
L the required date en the Acute Toxicity Analysis Form, and including the report date instead elthe acmal analysis date i
e The second error noted in 10/9'/ involved a missir.g quantity.
The third referenced error noted in the 4/98 report involved failure to include the pH information on the sport.

We have reviewed these occurrences and recognized the need for a more thorough l
audit and review of the monthly DMR report submittals. Also, we have notified our l
vendor that reports are due in a timely manner to assure prompt summary of the DMR data. To accomplish these objectives, we have implemented a new DMR Report Action plan.
