Gentlemen:

.This letter addresses the inspection of your facility at Windsor, Connecticut, conducted by Mr. R. L. Cilimberg and Mr. D. Lynn of this office on August 24-28, 1987, and the' discussions of their findings with you and other members of your staff at the conclusion of the inspection.

The purpose of this inspection was to review the fuel assem'oly' fabrication activities associated with Combustion Engineering (CE) and the implementation of the quality assurance (QA) program at your Windsor facility. The inspection included a review of tubing manufacturing and testing, fuel pellet manufacturing and testing, follow-up corrective action on previous inspection findings, and

)

evaluations by CE of leaking fue? at various facilities. Areas examined during the inspection and the findings are discussed in the enclosed report.

This inspection consisted of an examination of procedures and representative records, interviews with personnel, and observations by the inspectors.

.During the inspection, it was found that implementation of your QA program i

failed to meet certain NRC requirements.

The most serious finding was an unresolved item pertaining to an allegation that oversized fuel pellets had been put in trays containing acceptable fuel pellets by CE employees during 1985 and 1986.

We specifically request that you respond to this allegation.

This response should detennine whether the allegation is substantiated or not

'j substantiated, and describe corrective action taken or planned to prevent recur-l rence if the allegation is substantiated.

Other findings included the failure 1

to install protector caps on the machined ends of guide tubes and a door to j

the fuel and pellet stacking and loading room was propped open. Weaknesses in your QA program were recognized by CE when a stop work order was issued by the q

CE QA Manager on August 27, 1987.

An area of major concern is the safety

-significance of fuel pellets that do not meet dimensional specifications being installed in fuel rods which may pos "bly be in 13 operating reactors. The specific findings and references to the pertinent requirements are identified in the enclosures to this letter.

Please provide us within 30 days from the date of this letter a written statement containing:

(1) a description of steps that have been or will be taken to correct these items; 1
