,TMI-l' STATUS REPORT' FOR THE PERIOD JUNE 29 - JULY 15,1987

_Pjant Status As of,8:00 a.m. on Jub 15, 1987-, TMI-1 was' operating 'at full power with the. reactor coo! ant system (RCS) at normal operating temperature (579 'F average) and pressure at 2155 psig.

Facility Operations Summary During the rerort period, the plant. cnntinued to operate at full power since its return to power from the letdown coolers replacement outage.

No major problems were iden.'fied.

Item of Speci(1 Interest.

'High Envi roar. ental Monitor Reading and Correction to Previous. Report On June 29,.1987, in a letter to the Bureau of Radiation Protection, Commonwealth of Pennsylvania, the licensee reported additional anomalous values - at local area ' "Reuter-Stokes" environmental radiation stations.

The letter s tated:

"Eleva:ed exposure rates were recorded on Saturday, June 20, 1987 at the L Mill Street (Miadletown) Reuter-Stokes Station.

The anomalous readin as were detected between 13:45 EST [ Eastern Standard Time] and 15:00 EST and again between 21:45 EST and 22:00 EST of the same day, the v.tives ' ranging from 0.030 mR/hr to 0.14 mR/hr [30 micro-P.em/ hour to 140 micro-Rem / hour). It was determined that a faulty 300V [ volt]

battery, in conjunction with the severe lightning storms caused.the anomalous values.

Elevated exposure rates were also recorded on Friday, June 19, 1987 and Saturday, June 20, 1987 at the Falmouth (Collins Substation)

Reuter-Stokes Station.

The anomalous readings were detected between 17:00 EST of June 19 to 1:15 EST of June 20 and again on June 20 from 12:45 EST to 17 30 EST. The cause of the elevated values was again l

due to a faulty 300V battery in conjunction with severe lightning storms."

Also, the status report for the period May 4-15, 1987, erroneously reported the date of an anomalous reading at the North Gate Reuter-Stokes station. The date was listed as Tuesday, May 31, 1987, and it should be March 31, 1987.

The information was inadvertently misquoted from a licensee letter of April 22, 1987.

This was corrected as noted in the status report for the period May 29 - June 15, 1987.

Since then,. additional errors were identified for the May 4-15 report on this matter.

Readings for the North Gate and Alwine Farm Reuter-Stokes (recorded 'on April 20 and 24,- 1987, respectively) were erroneously.

reported as 0.19 and 0.025 micro-Rem / hour, respectively; and they should be 190 and 25 micro-Rem / hour, respectively.

Normal readings for' the environmental-monitors.ange from 5 to.10 micro-Rem /hr.

These abnormal readings and the licensee. explanation will be reviewed in a future environmental monitoring inspection.

Improper Support / Base Plate Installation On July 9,1987, the licensee verbally reported the improper installation of two support / base ' plates on the chilled water system for the control

' building (and control room) ventilation system. The. supports are located at the 330 foot elevation of the control building on adjacent supply and return lines along a vertical run of piping. On each support, one of four bolts.was not anchored to the wall.

Instead, the bolt heads were welded at the back of the base plate giving the appearance of proper installa-tion.

For one of the support plates,. no hole was drilled in the wall.

The licensee found this during inservice inspection of the supports. The licensee verified proper installation of the remaining three bolts on each support and they calculated a safety factor of approximately 2.4.

The licensee is in the process of documenting their engineering evaluation and have written a " material nonconformance report" on the matter.

In accor-dance with NRC staff guidance ' on.the matter, they do not plan to make l

repairs until the next plant shutdown.

c Tentatively, they have traced the faulty workmanship on one support to a welder who was at the site for a short period of time in the early 1980's.

The other support apparently dates back to new construction. The licensee 1s. evaluating any corrective action with respect to the implications of NkC Bulletin 79-02.

l THI-2 Allegation on Sleeping Operator On July 9,1987, the NRC staff at TMI-2 received an anomymous allegation that a licensed senior. reactor operator slept on watch and that apparently licensee management did nothing about it.

Licensee management also re-ported the finaing of " sleeping nests" generally in isolated areas of TMI-2 turbine building. The matter is currently under review by NRC staff at TMI-2.

The NRC staff at TMI-1 initiated a review of the matter for TMI-1. Init -

ially, the weekend and.backshift inspections coverage scheduled for later '

in the month were rescheduled 'for the weekend of July 11-12, 1987.

and' practices covering alertness and attentiveness to duties. for all plant workers' in addition ~ to - licensed operators.

NRC Staff Activities During the Period The NRC staff on site consisted of the sen'ior resident' inspector and one resident inspector.

On July 9,

1987, the' resident inspector staff concluded the routine monthly inspection of the licensee's activities.

The findings will -be l

documented in NRC Inspection Report No. 50-289/87-11.

Also, during this period, on Tuesday, July 7,1987, Region I issued NRC Inspection Report No. 50-289/87-10.

No violations were identified.
