NRR C0!EENTS ON FIRST' DRAFT OF SECOND QUARTER CY 1986 ABN0F. MAL OCCURREhCE REPORT TO C0hGRESS We have reviewed your memorandum of September 18, 1986, forwarding the first draft of the Abnormal Occurrence (AO) Report to Congress for the Second Quarter CY 1986.- Our con =ents on the write-ups for Abocnnal Occurrences are provided in Enclosure 1. Coments on the updated A0 write-ups are provided in Enclosure 2. Concents on the Other Events of Interest (Appendix C) write-ups, are provided in Enclosure 3. Af ter reviewing the Abnomal Occurrence write-ups and considering the most recent information on these events, we have concluded that neither the interruption of Decay Heat Removal at San Onofre Unit 3, the May 19, 1986 Plant Transient at l Palisades, nor the out-of-sequence Control Rod Withdrawal at Peach Bottom Unit 3, l 4 l meet the criteria for reporting as Abnomal Occurrences. We recomend that these events be reported as Other Events of Interest in Appendix C of the report. In addition, we are concerned that in general, the interpretations of criteria used to identify Abnomal Occurrences are steadily broadening. We believe that this is leading to an increase in the number of Abnomal Occurrences with a decrease in the norm of safety significance which in our view, is undesirable. Harold R. We do not belie'.e that the' March 26, 1986 event at. San Onofre Unit 3 meets the criteria for. reporting as an Ab_ normal Occurrence. In addition, ~ the information in the write-up.regarding the general decay heat removal. problem is rcre appropriately reported in Appendix B.as an update to-i Abnonnal Occurrence 80-5. 2. Specific courants on the write-up are prov'ided in the attached marked-up-I copy. _A0 86-9 OUT-OF-SEQUENCE CONTROL R0D W1THDRAWAL AT PEACH BOTTOM UNIT 3 1. We do not believe that this event alone meets the criteria for. reporting as an Abnonnal Occurrence. However, a trend tcward events similar to this' (i.e. significant personnel errors), may qualify for reporting as a serious deficiency in management or procedural controls. 2. Our editorial comments are provided in the attached marked-up Write-up. i A0 86-10 BOILING WATER REACTOR EPERGENCY _ CORE COOLING SYSTEM DESIGN DEFICIENCY. 1. Specific coments on the write-up are provided in the attached marked-up copy. A0 86-11 CONT.INUING MULTIPLE EQUIPMENT FAILURES AT PAllSADES 1. hRR agrees that significant improvements are needed in maintenance practices at palisades. However, we do not believe that the events described indicate a " serious delinquency 'in management or procedural controls in major areas." Therefore, we recommend that this event be reported as an "Other Event of Interest" in Appendix C. 2. The licensee actions section of the event write-up should be updated by l Region 111 to reflect information discussed at the 9/29/86 meeting with the NRC. 3. Other editorial conrnents on the write-up are provided on the attached marked-up copy. l i
