As a result of the inspect Jon conducted on October 14, 1987, and;'in accordance with 10 CFR Part 2, Appendh C - General Statement of Policy ant / Procedure for NRC Enforcement' Actions (1985), the following vjolations were identified.

2. Licehse Condition No. 10 requires the licensee to possess and use licerued '

material in accordance with the statements, representations, anir procedures contained in the licensee's application dated February 22, 1978. Item 6 on Page 2 of the licensee's Emergency"and Operating Procedures Manual submitted with the application requires that the Radiation Safety Officer perform an audit and inspection of all radiography personnel at 60. day in srvals. In addition,10 CFR 34.11(d)(1) requ}ns each licensee 'lo have a program that includes observations of the performance of each radiographer and radiographer assistant during an actual radiographic gperationatintervalsnottoexceedthreemonths.

Contrary to the above, the NRC inspector M$termined through a review of records and statements of licensee representatives that this requirement was not met. Specifically, no records were available to demonstrate that two radiographic personnel working near Dugger, Indiana have been audited or inspected during j.he duration of work at that site from August 10 to October 14,198'/,. nor were there any records to indicate that these individuals were audited or inspected prior to these months.

This is a Severity Level TV violation (Supplement VI).
