SUBJECT: DRESDEN INSPECTION REPORT 50-237/98014(DRP); 50-249/98014(DRP)

Dear Mr. Kingsley:

On April 3 through May 27,1998, the NRC performed an inspection of your Dresden Nuclear

Power Station. The enclosed report presents the results of this inspection.

During this inspection period, a safety-conscious approach characterized the overall conduct of

activities at the Dresden station. The operators generally performed well during one planned

reactor shutdown, two automatic and one manual reactor scrams, and five reactor startups.

However, multiple material condition deficiencies challenged the operators. For example, the

operators were required to scram the reactor in response to a loss of condenser vacuum caused

by a leaking cross-around relief valve.

The unavailability of the high pressure coolant injection (HPCI) system continued to be a problem

at Dresden during this inspection period. Operators twice declared the Unit 2 HPCI system 4

inoperable due to material condition failures. One of these failures involving the gland seal

leakoff condenser level switch has been a multiple, repeat failure with several occurrences over

the past year.

Continuing a theme documented in previous inspection reports, material condition failures and

deficiencies challenged and unnecessarily distracted the control room operators. In addition to

the reactor scram mentioned above, equipment problems caused difficulties in establishing a

cold shutdown condition, a loss of reactor feed pump ventilation during a reactor startup, and the

need for operators to establish attemate methods of reactor water level control following two

reactor scrams.

Continuing another theme from previous inspection reports, inadequately prepared procedures

and written instructions created adverse conditions that required station personnel to respond.

Examples included an automatic reactor scram due to an inadequate installation specification in -

' a modification, an unnecessary entry into a Technical Specification limiting condition for \1\ I

operation action statement due to poor electromatic relief valve surveillance instructions, and a

potential noncompliance with a license condition for main steamline radiation monitor alarms due

to another modification with incorrect specification .
