Dear Mr. Martin:
The purpose of this letter is to describe the self hnposed work hold at Wolf Croek Generating Station (NOGS) and to describe the conditions under which outage work activities will be resumed.
'Ihe curtailment of outage related activities was discussed Friday October 16, 1987 with Mr. Johns Jaudon of your staff and the Wolf Creek Resident Inspectors.
As a result of a series of events which occurred beginning October 10, 1987, NOGS management decided on October 16, 1987 to stop outage related activities to allow time for a managment review and evaluation prior to resuming outage related work. The events that lead to this decision were:
On October 10, 1987 a small anount of-resin 'was accidently-discharged into an enclosed rocm in the Rad Waste building when a-pressurized hose was disconnected during a-resin transfer.
- On the morning of October 14, 1987 a cmbustible mixture of gases-ignited while welding on - the Pressurizer level instrument upper-isolation valve.
On October 14, 1987 an electrician was electrocuted while cleaning electrical switchgear.
During the evening of October 15 and the morning of October 16, ESFAS actuations occured as a result of degrading battery bus voltage.
Voltage degradation was occuring due to one of the safety related electrical buses' (NB-02) being de-energized for outage work. This culminated in lake water contamination of all four steam generators through auxilary feedwater actuation signal and improper isolation of the auxiliary feedwater system.

Upon initial review of these four events WCGS nanagement decided to stop all outage related work in the plant not requ.tred to maintain the plant in a safe condition.
Surveillance, normal plant evolutions and priority corrective maintenance are continuing.
All work that was currently in progress was continued to a convenient and controlled stopping point. Three i
specific wrk activities that were continued through to cmpletion were the renoval of the three renaining fuel assemblies, Steam Generator clean l
up, and re-energizing NB-02.
These three activities were empleted to i
enhance safety and preclude degradation of the secondary system.

Project personnel wre briefed on the work stoppage and a nanagenent team i
i reporting to the Plant Manager consisting of plant staff, Quality Assurance, l
and Engineering and Technical Services personnel was formed to evaluate the series of events.
Individuals fran the plant staff, Independent Safety i
Engineering Group, Quality Assurance and Nuclear Plant Engineering were I
assigned to specific teams to investigate the four events.
The four events are being investigated individually and will be reviewed together to s%
what, if any, comen root causes can be identified.
I The NCGS nanagement team has reviewed the scope of the outage work stoppage l
and evaluated the conditions under which work will be allcwed to resume.

three phase program has been developed to control work resumption.
The j
l first phase identified jobs which should be exmpted fran the work l
l stopinge.
The second phase established a controlled process to allow resumption of non-safety related jobs and jobs that will restore safety systems and cmponents or otherwise enhance safety.
The third phase was to establish conditions under which the renninder of the outage related wrk can be resuned.
I Phase 1 The following jobs have been evaluated as having little or no potential for complications and therefore should be exempted fran the work stoppage.
Those jobs will be restarted after steps 1, 2,
and 3 described in Phase 2 beloa have been cmpleted.
- MWATS 'Ibsting - Testing of notor operated valves in accordance with I&E Bulletin 85-03.
- Errosion/ Corrosion Measurements - The UT testing and/or visual inspection of selected piping to identify possible pipe Wall i
l thinning.
- Fire Penetration Inspection /ReWor)5 - The task force efforts for inspecting fire penetration seals and tworking seals found to be in nonempliance with the requirements.

Required inspections,
- rework, and Turbine Generator Work reass ably of the Turbine Generator which do not require changes in system lineup.
- Service Water Pumps - Impeller replac m ent and restoration of the Service Water System.
Fuel Shuffle in the Spent Fuel Pool - Shuffle of the fuel assablies.
in the Spent Fuel Pool only.
No novment of fuel into the Reactor until full resumption of outage.
i Phase _2 The following criteria have been established to control the resumption of non-safety related work and selected safety related jobs that restore safety systeis or otherwise enhance safety system availability / reliability.
Wolf Creek will resume a limited anount of work under these controlled corx31tions until the investigations have been cmpleted on the four events and any necessary Jamediate corrective actions have been empleted.
We conditions under which work will be allowed to resum on a listited basis are sumerized below:

1.
Outage jobs will not be resumed until each work group has j
l conducted meetings with their splayees and contractors m phasizing the need to do the job right, the need to follow l
procedures, and the need to fully understand the job requirennnts and the safety precautions before cmunncing work.
2.
Each job will be " walked-down" to provide added assurance that the job can proceed safely.
3.
The job cannot resum without specific approval of the Plant Manager.
4.
% e only safety related outage work that will be allowed is work that will restore safety systms or otherwise enhance safety systems availability / reliability (i.e.
safety related l
I inspections / testing activities).
5.
Non-safety related work will be allowed after satisfying criterion 1, 2, and 3 above.

three criteria are met:
1.
Ccap.letion of the investigations into the four events.
2.
Ccapletion of all inmediate. corrective actions identified as a result of the investigation end management reviews.
Iong term corrective actions or enhance ents will be scheduled.
3.
Notification of NRC of NOGS's intent to resume outage.
It must be emphasized that throughout this current limitation on work, Wolf Creek's primary attention will be devoted to maintaining the plant in a safe condition.
Therefore, the plant staff will take any action necessary to maintain the plant in a safe condition and will continue with activities required to renain in ecmpliance with the Technical Specifications.
