Attached is a detailed response to violation 482/8720-01 which was documented in the Reference.

Violation 482/8720-01 concerns a failure to enter Technical Specification 3.0.3 when both trains of the control room emergency ventilation system were inoperable when a door was blocked open.

Contrary to the above, on August 20, 1987, the NRC inspector observed both.

1 trains of'the control room emergency ventilation system inoperable when Door

36171 was. blocked open preventing the control room' from being, pressurized to 1

]

greater than or equal.25 inches of water.

Reason For Violation:

i On August'20,

1987, at approximately 1330 CQU _ the NRC Resident. Inspector i

found Door 36171, which separates the ControY) Room from an electrical chase l

area, propped open for rework of penetrations by Facilities and Modification (F&M) personnel.

The Shift Supervisor was awaie of the rework activitf,1 but was unaware that it required thq door to be propped open.

This Technical Specification violation was reported,to ythe NRC in LER 87-034-00.

,j Corrective Steys Which Have Been Taken and Results Achieved: /

The Shi ft Supervisor promptly took compensatory measures upon notification that the door was propped open to restore the operability of both Control Room Emergency Ventilation Systems.

Corrective Steps Which Will_ Be Taken To Avoid Further Violations:

f In order to prevent the recur'rence of this event, the following corrective actions have been implemented:

(1)

Permanent signs have been posted on Door 36171 and two other similar doors informing personnel that the doors are part of the Control Room -

pressure boundary and the Control ?oom must-be contacted prior to initiating any work that would require propping the doors open.

.F&M Engineers have been instructed to include precautionary statements j

into their work packages to emphasize pressure boundary considerations -

prior to' starting work.

F&M personnel, including Contractors,. have been instructed to contact-

. Control Room personnel when work could affect a pressure boundary.
