SUMMARY

OF MEETING ON STATUS OF RECOVERY PLAN l

The NRC staff met with the senior management and staff of the Philadelphia Electric Company (PEco) in Bethesda, Maryland on July 15, 1987 to receive a j briefing from PECo on the status of their recovery program for operations at i the Peach Bottom Atomic Power Station (PBAPSI. The PBAPS has been shutdown by  ;

an NRC Order issued on March 31, 1987 because of the problem of sleeping operators, and because of a history of other occurrences which led NRC to question the management of the facility. The issuance of the order was noticed in the Federal Register on April 8, 1987(52FR 11386-11388).

The principal speakers included PEco's Chairman, Mr. Lee Everett and President, Mr. John Austin, the NRC staff Deputy Executive Director for Operations, Mr. Jim Taylor, the Director of the Office of Nuclear Reactor Regulation, Dr. Thomas Murley and the Region I Administrator, Mr. Bill Russell. A list of attendees is provided in Enclosure 1. The discussions in the meeting were transcribed. The following summary notes the most salient points of the discussion and the associated page of the transcript in parentheses. The licensee's slides are provided in Enclosure 2 and a copy of the transcript, annotated by the licensee, is provided as Enclosure 3.

Mr. Jim Taylor opened the discussions by noting that NRC is conducting its own investigation and at this time is not ready to discuss the results or status of that investigation. The licensee should not take the staff's silence or comments, either as agreeing or disagreeing with PECo's course of action.

Mr. Lee Everett's Presentation:

Mr. Everett, Chairman of PECo, introduced the four PECo speakers and their roles. John Austin, President of PECo was identified as the leader of the recovery program. Mr. Dick Smith, Manager of the Peach Bottom facility, would discuss what PECo 'ound at the plant and what PECo was doing about it.

Mr. John Kemper, Senior Vice' President, would present an analysis of the plant situation and the preparation for the restart program.

Mr. Everett indicated that he considered the shutdown to be the most devas-tating event in his history of 37 years with the company. He indicated that the program to be described in this meeting would be "a total corporate commitment not only to return Peach Bottom to operation, but to bring it up to a standard of excellence that we can be proud of in the industry."

Mr. Everett described efforts to find the root causes and means to eliminate them which included an in-house team headed by John Austin, a special committee ,

appointed by.the Board of Director to r>versee the entire' operation and the '

services of the Management Analysis Corporation (MAC).

Mr. Everett discussed the basic problem, a lack of leadership at the plant, the operator evaluation screening and retraining program, procedures, ad-ditional personnel, a new code of professional conduct, and past criticisms of NRC and INP0(7-9).

1 Mr. John Austin's Presentation: '

Mr. Austin's objective was to sumarize the actions taken since the shutdown order, the conclusions reached and an overview of the corrective actions (12).

In addition. to the MAC investigation, Mr. Austin noted that INPO had fomed, at PECo's request, an Industry Review Committee (13)..that Admiral Wilkinson had been retained as a consultant to the Board of Directors (14), and that PEco's Security Division was investigating (14).

Mr. Austin indicated that the Project Manager- Team's efforts now ' include 70 i people and that this could grow to 200 and that the overall effort has been.

identified as a Commitment to Excellence (CTE) plan (16).

Mr. Austin then discussed the root causes(17) and history of plant operation and recent actions taken(17-36).

Mr. Austin noted that of the 36 operations personnel at the time of the l shutdown, PECo had decided that 12 would not return to power operation (27). I Mr. Austin noted that Unit 3 would begin a one-year outage for large pipe replacement on October 15(28), and compared the Limerick and PBAPS J experiences (28-31).

l Mr Austin indicated that the action plan for recovery and restart would be )

, filed the first week of August and that PECo expected to be ready to restart i one unit near the end of October (32).

Mr. Dick Smith's Presentation:

Mr. Smith discussed his actions since assuming the position of manager of the  ;

Peach Bottom plant on May 6(34-36).

Mr. Smith stated that "the site personnel do have a general positive attitude.

They readily accept that there is a need for change and they are eager to have change. I think there has been progress"(37).

Mr.' Smith discussed procedures, health physics, radioactive waste,. plant l decontamination, repainting, labeling, maintenance, security (37-41).

Mr. Smith also discussed the forthcoming operations organization and personnel, .

including the Shift Manager concept, career ladders, retraining, and training j for future operators (42-61).

l Mr. John Kemper's Presentation:

Rr. Kemper noted that PECo had spent the last three months deveJoping this q commitment to excellence program and now has over 800 tasks fo.r the recovery i plan.

Mr. Kemper discussed the root causes(63-64); communications (65); supervisory skills (65); surveillance testing on all shifts (66); externai relations (66);

and the improvement program (67), App R(70), hydrogen water chemistry, condenser tube plugging, valves, the secondary alarm system (71); human factors in control {

room, preventive maintenance (72); new hardware and facilities (73-78); quality l assurance, radwaste, health physics, facilities, security, consnunications j (78-85).

Closino Comments:

1 Mr. Everett indicated that prior to the shutdown, PECo had implemented a program that would have improved performance in an evolutionary way, but it is now recognized that the evolutionary program was not enough. He indicated that PECo realizes it must improve Peach Bottom in a revolutionary way. . . to make a quantum leap into excellence. . . during this shutdown period (85-86).

Mr. Taylor commented regarding whether PECo is satisfied that when PEco does get to restart, that they won't have some of the same problems with the {

non-licensed personnel out on the floor. Mr. Smith responded that the {

non-licensed operators would also receive re-training (87-88). 1 Mr. Taylor noted for PECo's consideration that there had been serious discussion within the NRC staff of proposing a rule that the entry of an NRC )

inspector into the plant would not be announced by the licensee within the i plent(88-92).  !

Dr. Murley noted that at some point NRC would get concerned about what the l licensee was doing to Limerick if they started draining too many people from i

, Limerick. PECo responded that utilization of Limerick people would be limited )

and on a case-by-case basis (92-94).  ;

i Dr. Murley noted that the shift managers would be key people in the 1 organization with respect to ensuring that improved attitudes on shift are maintained after restart (95).

Dr. Murley focussed on ensuring that, in fact, a change in attitude would be effected and on measuring the readiness of the team to perform their functions (96-99).

Mr. Russell questioned the extent to which PEco had factored into their personnel screening process the results of the PECo investigative work as it relates to individuals (99-101).

Mr. Russell asked about the preoperational surveillance testing program and an approach to restart (101-102).
