As requested in Reference (a), Baltimore Gas & Electric is providing the following response to the subject Notice of Violation. Two Severity Level IV violations were assessed against our radioactive waste shipping program.
1.
Corrective Aglion Taken and Results Achieved On April 29, 1988, a slightly-contaminated, large metal calibration block was shipped offsite by the user who failed to contact the Materials Processing Unit (the authorized radioactive material handling group) prior to shipment.
Therefore, appropriate radiological surveys were not performed and the appro-priate shipping requirements were not met. As a result, the block was shipped offsite without the proper labeling and shipping papers.
The second violation occurred as a result of the first. IInd the Material Processing Unit been aware of the need to ship the contaminated block, the receiver's license would have been verified per Radiation Safety Procedure 2-204.
In this case, the receiver was authorized to accept contaminated material and its license is maintained on file in the Materials Processing Unit.

Document Control Desk March 16,1989 Page 2 Immediate corrective actions were initiated to ensure that all applicable groups were informed of the violation. All Nuclear Energy Division supervisors were informed of the need to have all materials removed from the radiologically controlled area surveyed prior to shipment offsite. Additionally, the users of the block have been instructed in procedural requirements concerning removal of '
material from the radiologically controlled area and shipment offsite. General
-Orientation Training has also been revised to make sure that these requirements are known to all site personnel. Both the initial training and requalification j
training lesson plans have been revised.
2.
Corrective Steos Which Will be Taken to avoid Further Violations The corrective actions outlined above will provide training to personnel involved in moving material from the radiologically controlled area. This action should prevent recurrence of the violation.
3.
Date When Full Comoliance Will be Achieved I
l The Nuclear Energy Division supervisors were informed of the event by a memo i
dated January 12, 1989.
The group involved in the incident received their training on February 21, 1989.
- The General Orientation Training lesson plan was updated on March 8,
1989.
Training provided after that date included contaminated material handling requirements. It will take approximately one year for all plant personnel to be re-qualified with the updated procedure.
