Safety and Security Supervisor Post Office Box 1607 Terre Haute, IN 47808 Gentlemen:
4 We have reviewed your application dated February 19, 1988, requesting a renewal to NRC License No. 13-17751-01 and find that we will need additional information as follows:
1.
Your application states Post Office Box 1607, Terre Haute, Indiana as your place of use. However, your license currently lists North 13th Street Road, Terre Haute, Indiana as your place of use.
Please state if the j
address currently listed on your license is the correct place of use.
Note that a post office box is not an acceptable address fnr the location of use.
2.
In Item 7 of your application, you state that employees will be trained at the time of installation or given equivalent training by Michael J. Tolin.
For the equivalent training given, please provide an outline of the training and show its equivalency to the training offered by the device installer.
3.
In reference to your safety procedure for the service of the radiation gauges, please modify the procedures to include performing checks to make certain the gauge is working properly after reassembly.
4.
In regards to the use of your survey meter, please submit the following information:

Please note that your survey meter, Texas Nuclear, Model No. 2651 must be able to measure at least one through 200 milliroentgen per hour.
Please confirm.

Confirm that your will have a calibration chart or graph that shows the results of the calibration, the date of the last calibration, and the due date for the next calibration affixed to the survey rate.
