Inspection Report No. 70-1100/89-01 This refers to the special inspection conducted by Mr. J. Roth of .this office on January 30 - February 3,1989, for the purpose of reviewing your manufacturing operations,: criticality safety, the status of the Task Force Report and implementation of.the Nuclear Fuel Integrated Improvement Plan with regard.to the development of the Radiation Protection Instructions. The inspection consisted of selected examination of procedures and representative records, interviews with personnel and observations by the inspector.

. Seven apparent violations of NRC requirements were identified. You are required to. reply to two of these in accordance with the enclosed Appendix A. That reply is not subject to clearance by the Office of Management and Budget under the Paperwork Reduction Act of 1980, PL 96-511. Five of the seven apparent violations identified met the mitigative tests described in 10 CFR 2, Appendix C; consequently, these apparent violations were not discussed in the Notice of  ;

Violation enclosed with this letter. These apparent violations concerned adherence to a posted nuclear criticality safety sign; assurance that inter-

-spersed moderation (water) could not enter the fuel rod box storage area; maintenance of adequate face velocity on chemistry laboratory hoods; maintenance of the required exclusion zone around a.new set of pellet storage shelves; and, adequate measurement of effluent releases from the FA-3 ventilation system stack. The establishment and implementation of your long-term corrective actions for these apparent violations will be reviewed during a subsequent inspection.

During the inspection, the inspector was concerned that you had not issued the reviewed and approved Radiation Protection Instructions (RPIs) to the production or health physics staffs for implementation. Since implementation of these RPIs was. required to complete corrective actions on previously identified enforcement items, a management meeting was scheduled and held subsequent to this inspection, on February 16, 1989, for the purpose of resolving those concerns. The discussions that took place during that meeting are summarized in a meeting report sent to you dated March 7, 1989. j I
