SUMMARY

OF FEBRUARY 14-17, 1989 AUDIT OF REACTOR PROTECTION SYSTEM UPGRADE 50.59 REVIEW On February 14 through 17, 1989, the staff met with Connecticut Yankee Atomic Power Company (CYAPCO) and the Foxboro Company (CYAPC0 vendor for reactor protection system (RPS) instrumentation) to review the RPS upgrade implemented during the 1987 outage. The staff stated that its >urposes were to:

1) review the hardware and software installed during t1e Phase I of the RPS upgrade,2)reviewthehardwareandsoftwarerelatedtothePhaseIIchanges and 3) provide a SER for the Foxboro instruments being used at Haddam Neck.

In addition, the staff hoped to address the 50.59 analysis and whether the RPS upgrade was performed correctly under 50.59.

The staff felt CYAPC0 and Foxboro were well prepared and had a good working knowledge'of.the equipment'and its installation. As part of the audit, CYAPC0 walked through a reactor trip signal from sensor to trip.

In general the staff concluded that the equipment and installation were properly implemented and CYAPCO has demonstrated " functional equivalence" of the digital system to

.the original' analog. The staff did however have several open items which they felt CYAPC0 needs.to address:

Several errors were identified by Foxboro in the software. There seems to be no paper trail that concludes these items were corrected.

The staff believes an independent review should be performed to verify that a11' identified software errors were corrected. The review should include at least the four modules being used by CYAPCO but can include all modules produced by Foxboro,

The staff needs CYAPCO to document the software reliability program being used by Foxboro, 1

3)

The staff'needs CYAPC0 to state that the new system is functionally equivalent to the original system (comparison of digital curves versus analog curves),

4)

CYAPCO needs to demonstrate that the EMI testing by Foxboro bounds the EMI spectrum at the Haddam Neck site, 5)

CYAPC0 needs to document that the fault testing of L2CR (relay card) was by a transformer at 20 amps or greater, 6)

CYAPCO needs to document that the seismic test performed for the L2CR envelopes the seismic spectrum at the Haddam Neck site, 890316034{

The staff stated that with the resolution of the above, the staff can conclude that the Phast'I and II.RPS upgrades at the Haddam Neck site are adequate.

However, the staff is still of the opinion because.of the dramatic change in i

technology, the RPS upgrade should not have been implemented under 50.59.

The staff proposed to issue a safety evaluation in 4 to 6 weeks.
