Dear Mr. Voit:

This is to request information about three incidents in which technical discrepancies were found with waste process / containers delivered by L. N.

Technologies Corporation for disposal at the Barnwell site. These incidents occurred on June 6, 1988, October 11, 1988 and October 13, 1988 and are described in the enclosed sumary reports.

Please provide the following information for each incident:

1. The cause of the event in more detail than that provided in the summary report.
2. The testing that was preformed on the liner after it was returned to the generator.
3. The disposition of the waste in the defective liner, e.g., a description of what was done to the defective liner to make it acceptable for burial, the current location of the waste and, if applicable, the date of disposal and the disposal site.

We request that you provide this information within 30 days after receipt of this letter.

@CRIPT. OF INCIDENT: DHEC Inspectors identified a swollen and distended liner-lid due to gas generation. The liner was banned from burial at Barnwell, depressurized, and returned to Perry,-OH for testing. Samples of the gas indicated high concentrations of carbon dioxide and methane present. . It was further determined that strong oxidants.mmy have caused the chemical reaction responsible for this event.- '

OF INCIDENT: DHEC Inspectors performed a punch test on the liner, and encountered a resin bead jelly-like substance which flowed from the hole after 1-2 minutes. The waste also contained 7.6% Picolinic Acid, which in an unsolidified form violates license agreements. The liner was banned from Barnwell, and returned to Lycoming, NY for testing. It was determined that improper mixing of the chemicals caused the jelly-like substance to form on the inner wall of the liner. Chunks of the substance then broke free from the wall during mixing, becoming randomly distributed.throughout the media, forming jelly pockets after solidification.

'ACKAGE TYPE: STEEL LINER iESCRIPT. OF INCIDENT: DHEC Inspectors performed a punch test on the liner, and drained out 5.94 gallons of free liquid. Acceptable limit for a 147 cuFt liner is 5.5 gallons, therefore, the free liquid content in the liner exceeded limits by 0.44 gallons.
