SUBJECT:

BEAVER VALLEY 2 TEAM INSPECTION OF INITIAL CRITICALITY.AND LOW POWER OPERATION As a result of the July 8, 1987 Comm ssion meeting to consider a full power license for Beaver Valley 2, the Commission "equested the staff to provide a written sum-mary of the licensee's low power operating experience at Unit 2 before Commission action authorizing the staff to issuo a full power operating license. We are planning a team inspection of activi*.ies at Unit 2 starting approximately two days before criticality through operation up to 5% power to provide the bases for such a summary.

The inspection team will consist of 1.he following personnel:

Lowell Tripp, Team Leader Jim Beall, Beaver Valley 2 SRI and Assistant Team Leader Peter Wen, Startup Specialist, DRS Skip Young, Beaver Valley 1 SRI (part time)

Steve Pindale, Beaver Vality 1 RI (part time)

Len Prividy, QA Specialist, DRS Andra Asars, Haddam Neck R1 Peter Tam, NRR Project Maneger (part time)

A draft copy of the inspection plan is enclosed. Round the clock coverage will be provided of final preparations for entering Mooe 2 (initial criticality), ini-tial criticality, low power operations and testing, and. completion of prerequisites for entering Mode 1 (75% power). A special inspection report will be issued at a later date. However, to provide the operating experience summary in a timely manner, each inspector will be required to provide a SALP-type evaluation of their observations and inspection experience before they leave the site. Using these inputs, an overall summary evaluatior, will be developed by the tear leader to en-close with the Commission paper (note reference to such ti report in the enclosed draft paper) within three days ofter the licensee finishes this phase _of startup.

1. Plant Operations 1.1 Control Room Activities Evaluate control room professionalism.

Observe shift turnovers, and briefings.

Observe operator perfotaance/ interview operators to assess adequacy of operator knowledge.

Review logs and standir,g orders.

Review procedures, drawings, and other material available to opera-tors in control room.

Observe approach to initial criticality, ongoing plant evolutions, and startup testing.

Evaluate adequacy of interface between testing and operations.

1.2 Control Boards and Annunciators Review energized annunciators.

-- Observe acknowledgement of annunciators / response by operators.

-- Evaluate adequacy of information available to operators.

1.3 Post-Trip Reviews and Incid(nt Response (observe and evaluate the fol-lowing as applicable)

-- Trip / incident recovery actions.

-- Licensee post-trip review process including restart authorization.

Reporting per 10 CFR 50.72 (and 50.73 for previous events).

Unit Off Normal Reports.

Review planning and coordination of ongoing maintenance / surveillance:

activities.

Sample ongoing actions with respect to adherence to administrative controls for taking syctems/ components out of service and restora-tion (alignments, tagg'ng, etc.) and operator awareness of-plant conditions.

1. 5 Independent Verification Review program for independent verification of positions of safety related components /sysiens.

Inspect sample of surveillance / maintenance procedures for valve and electrical lineups to include observation of independent licensee verification activitiet.

2. Maintenance Review cample of work orders for evidence of proper planning, authoriza-tion and control as well as description of work, QA/QC inspection in-volvement, etc.

In additien to 1.4 above, observe a sample of actual " window of oppor-tunity" maintenance work activities.

Review a sample of post-maintenance testing to include:

Selection of test (s) - how determined? by whom?

Test and acceptance criteria.

Adequacy of post-maintenance testing for demonstrating operability.

3. Testino and Surveillance Observe and evaluate startup testing such as low power physics tests to include conduct of testing, interfaces, and evaluation of results.

Review licensee overall systun to ensure completion of required surveil-lances including responsibilities, planning and scheduling, tracking, etc. In particular, review licensee method for identifying and control-11ng those surveillance testi required to support mode changes.

Evaluate review / control of OIT deficiencies.

Evaluate adequacy of control and planning of daily activities (including-morning meeting).

Review mechanisms used for f'ow of'information to' management.

Commissioner Bernthal requested the staff.to follow carefully

S5841, provides that action.of the Commission shall be deter-mined by a s " majority vote of the members present." Commissioners Carr and-Rogers were not present when this item was affirmed.

Accordingly,.the formal vote of the Commission was 3-0 in favor of the decision. Commissioner.Carr, however, had previously indicated ~that he would approve this paper.and had he been present he would have affirmed his prior vote. Commissioner' Rogers did not participate.
